fix(authz): hard-gate every credential export and CLI-config write (GHSA-5926-2w35-7h4q) (#12600)
* fix(authz): hard-gate every credential export and CLI-config write
GHSA-5926-2w35-7h4q: `POST /api/providers/{id}/claude-auth/export` and
`.../codex-auth/export` gate on `requireManagementAuth(request)` with no
`alwaysRequireAuth`, and neither path was in ALWAYS_PROTECTED_API_PATHS. Under
`requireLogin=false` — the local-first default — both fail open, so anyone who
knows a connection id downloads the operator's raw Claude/Codex OAuth
access_token / refresh_token (plus the Codex id_token).
This is the third recurrence of one class. GHSA-mghq-58h3-qcqj added
/api/db-backups; GHSA-v7g9-7f55-5g46 added the /api/settings/*-json siblings
mghq had missed; these two are the siblings both missed. So the fix is written
against the class, not the two reported routes.
Sweeping every route that hands out stored credentials, dumps captured traffic,
or writes the operator's CLI config turned up four more on the fail-open tier:
- GET /api/logs/export — dumps call_logs (prompts and responses) and proxy_logs
for up to 168h.
- /api/cli-tools/codex-profiles — GET leaks the operator's account label; PUT
writes attacker-supplied auth.json and config.toml straight into the host's
Codex CLI config. Its only guard is ensureCliConfigWriteAllowed() with no
targetPath, which checks CLI_ALLOW_CONFIG_WRITES — default true. Paired with
the POST that stores an arbitrary profile, that is: save a profile holding the
attacker's auth.json, apply it, and the operator's CLI now runs on attacker
credentials (or, via config.toml, an attacker base URL).
- {claude,codex}-auth/apply-local and providers/agy-auth/apply-local — write a
stored credential into ~/.codex/auth.json and
~/.gemini/antigravity-cli/antigravity-oauth-token.
The traffic-inspector HAR exports were already covered by LOCAL_ONLY.
Routes with a dynamic segment cannot be expressed in the exact/prefix list — a
`/api/providers/` prefix would hard-gate the whole provider surface and break
every keyless install — so this adds ALWAYS_PROTECTED_API_PATTERNS, mirroring
the existing LOCAL_ONLY_API_PATTERNS, and `isAlwaysProtectedPath` consults both.
The apply-local routes get ALWAYS_PROTECTED rather than LOCAL_ONLY on purpose:
it closes the anonymous hole without breaking an operator driving the dashboard
through a tunnel.
Deliberately NOT adding `{ alwaysRequireAuth: true }` at the handlers. Tier 2 is
the architecture's designated mechanism and the guard runs before the handler; a
second copy of the same decision inside each route is exactly the kind of
duplicate that drifts out of sync (cf. the dashboardCsrf prefix scan that had to
be unified in #11417).
tests/unit/authz/credential-export-always-protected.test.ts — 5 tests, red
before the fix. Written as an inventory of the whole class rather than two more
assertions, plus negative cases: the neighbouring provider routes must stay on
MANAGEMENT, and a connection id containing a slash must not slip past `[^/]+`.
openapi.yaml marks the seven newly-gated operations `x-always-protected`, and
openapi-security-tiers.test.ts now resolves `{param}` placeholders so it can
validate the pattern entries too.

The new tests/unit/authz/credential-export-always-protected.test.ts covers
src/server/authz/routeGuard.ts, so check:mutation-test-coverage --strict fails
until it is listed — its mutant kills would not count otherwise.
Inserted in place (no re-serialization: a JSON round-trip on this file reorders
~10 curated entries that are already out of alphabetical order, cf. #11438).

* docs(api): document every implemented route in openapi.yaml (276 -> 692 paths)
Follow-up nº 3 of the 2026-08-31 docs audit: 416 implemented routes had no
OpenAPI entry (gamification, radar, skills, webhooks, mcp, a2a, tunnels,
version-manager and plugins were absent entirely). Adds a minimal, honest
entry for each — real methods parsed from every route.ts's exports, a group
tag and a neutral path-derived summary; no invented semantics. Rich schemas
remain hand-curated in the existing entries.
Generated by scripts/ad-hoc/gen-openapi-missing-paths.mjs, which enumerates
routes with the same lib check:api-docs-refs uses — the spec now covers
692/692 real routes and the gate verifies every spec path has a real route.
* docs(api): security tiers on generated paths, regenerated API skills, size baseline
The first CI round caught three real contract gaps in the generated coverage:
- Generated operations on LOCAL_ONLY routes now carry x-loopback-only (and
x-always-protected for ALWAYS_PROTECTED_API_PATHS), resolved through the real
src/server/authz/routeGuard.ts at generation time. The
openapi-security-tiers guard now also accepts LOCAL_ONLY_API_PATTERNS —
param-shaped routes (/api/providers/{id}/login) are classified by regex in
the runtime and were invisible to the prefix-only check.
- The API agent skills are generated FROM the spec: 18 SKILL.md files
regenerated via generate-agent-skills --apply so the generator stays 46/46.
- src/app/docs/lib/openapi.generated.ts grew with the spec (171 -> 1347 lines,
emitted by gen-openapi-module): frozen in file-size-baseline.json with a
_rebaseline justification — shrink by slimming the spec, never by editing
the generated module.

The release-green pre-flight (Solution C) previously covered only a subset of the
gates that run exclusively on the release PR (PR→main), so reds still accrued
silently on release/** and surfaced in ~40-min layers at release time (v3.8.34:
3 CI rounds — CodeQL sanitization, then the fail-fast Quality Ratchet revealing
openapi then cyclomatic-complexity one push at a time, plus zizmor/integration).
Now check:release-green reproduces the COMPLETE release-PR gate set and reports
EVERY red in one pass (collected, not fail-fast):
- New DRIFT ratchets (report-only, rebaselined at release, never block):
cyclomatic complexity, dead-code, type-coverage, compression-budget,
openapi-coverage, workflow-lint (zizmor), codeql-ratchet.
- New HARD gates (real defects): docs-all (fabricated-docs strict + i18n mirror
sync) and the integration test suite (gated behind !--quick).
The only release-PR gates it still cannot reproduce locally are GitHub-side CodeQL
semantic analysis and SonarQube/SonarCloud (external services).
The nightly-release-green workflow and /green-prs inherit the expanded coverage
automatically (they invoke this script), so cycle drift is now surfaced
continuously and the release PR is green on its first CI run.

Before: if body.model supports vision, the vision bridge skipped image processing entirely. Non-vision combo targets would receive raw images they can't handle. After: before skipping, check if the model has a model-combo mapping. If it does, process images through the vision bridge regardless of body.model's native vision support. - Add checkModelHasComboMapping() helper (dynamic import, failsafe) - Add checkModelHasComboMapping dep to VisionBridgeDependencies (testable) - Guardrail preCall: check combo mapping before early-return on vision support - Add VB-S11 / VB-S11b tests * fix(vision-bridge): only process images when some combo targets lack native vision Optimization per code review: instead of always processing images when a combo mapping exists, resolve the combo targets and check each target model's native vision support. Only invoke the vision bridge when at least one target model does not support vision. - Replace checkModelHasComboMapping() with shouldProcessImagesForComboModel() - When combo has ComboRefStep targets, conservatively process images - When all targets are model steps with native vision, skip processing - On errors, process images (conservative fail-safe) * fix(combos): repair context handoff ordering and add per-model timeout Root cause: recordSessionModelUsage was called BEFORE getLastSessionModel, so prevModel always matched the current modelStr — handoff summaries were never generated when auto-routing switched models. Fix: call getLastSessionModel first (captures actual previous model), generate handoff on mismatch, then record the new model for next time.
