feat(auto-combo): declare observed reliability as a scoring factor (#12317)
The weight table said stability accounts for "low latency stdDev / error rate". Grep errorRate in scoring.ts and you find it declared on ProviderCandidate and read nowhere — while combo.ts pulls 24 hours of usage history behind a ten-sample floor, falls back to real-time metrics, and hands every candidate an errorRate the scorer ignores. Two candidates, one failing 1% of calls and one failing 99%, scored identically at 0.459486. This declares reliability as a sixteenth factor: 1 - failureRate, using the same formula, field precedence and rate-bounding speedRanking.ts already applies, so a corrupt reading means "nothing observed" rather than "fails every call". It ships at weight 0, leaving the ranking unchanged to the digit — the honest default, since which weight this deserves is a product call backed by traffic the author does not have. Two declared-but-silent factors already ship (cacheAffinity, resetWindowAffinity), so the pattern is not new. The stability row now describes what that factor actually computes: latency variance. The rest is the mechanical 15 → 16 across nineteen documents and the forty-two llm.txt mirrors — sourced from check:docs-counts rather than a grep, the first real use of the gate #12316 extended. Protected-surface note: this PR touches AGENTS.md, llm.txt and its 42 mirrors, and skills/omni-combos-routing/SKILL.md. Every changed line in those 45 files is a digit substitution and nothing else — masking all digits makes the removed and added lines identical, with no sentence added, removed or reworded. Reviewed and approved on that basis before merging. Verified on the author's rebased head: check:docs-counts green (the gate that now enforces the count this PR moves), typecheck:core clean, and 71/71 focused tests across scoring-reliability-factor, combo-scoring-weights-schema-coverage, check-docs-counts-sync, lkgp-enabled-context, intelligent-routing-options and the combo-matrix auto integration suite. Thanks @maxmad64bis — shipping the factor at weight 0 and saying plainly that the weight is someone else's call is the right way to land this. |

refactor(sse): ExecutorRegistry — route executor lookup through a runtime registry (R0.3) (#10633)
* test(sse): golden characterization of the executor map before the R0.3 registry refactor Freezes the 137-entry provider-id → executor mapping (class, provider identity, backing PROVIDERS config), the no-shared-instances invariant, and the getExecutor() dispatch rules (memoized DefaultExecutor fallback, cloud-agent guard #6699, search-provider guard #10274) as stable JSON snapshots. The upcoming ExecutorRegistry must keep both snapshots byte-identical. * refactor(sse): route executor lookup through ExecutorRegistry (R0.3) Adds open-sse/executors/registry.ts (Map-based registry mirroring translator/registry.ts): the built-in table in executors/index.ts stays declarative, every entry is registered at module load, and getExecutor()/hasSpecializedExecutor() resolve through the registry. DefaultExecutor fallback, its memoization, and the cloud-agent (#6699) / search-provider (#10274) guards are unchanged. Also fixes a latent lookup leak: the old object-literal lookup treated Object.prototype names (constructor, toString, ...) as specialized executors; the Map registry resolves them to the DefaultExecutor fallback like any unknown provider. Parity proof: executor-map golden (137 entries, byte-identical before/after), check:known-symbols green, 1018 tests across the 65 executor test files green. Docs: OPEN_SSE_ARCHITECTURE factory section corrected (it claimed generation from providerRegistry). v3.8.40 cycle integration → main. All test gates green (Unit/Integration/Coverage/Node-compat/Quality-Ratchet). The only red check, 'PR Test Policy', is the test-masking heuristic firing on the cumulative ~57-commit release diff (legitimate assert consolidations already reviewed per-PR — Gemini CLI removal #5246, retired GPT models #5280, provider catalog refreshes); overridden with --admin per the documented release-PR convention. CodeQL/SonarQube advisory scans non-blocking; #5278's code already passed CodeQL on main. Homologated on VPS 192.168.0.15 (v3.8.40 healthy). |

Release v3.8.37 (#5053)
* chore(release): open v3.8.37 development cycle * chore(ci): harden release flow — ratchet decoupling, fast-path drift gates, build-scope guard, heap default (#5054) Implements improvements 1-4 from the v3.8.36 release benchmark (_tasks/release-bench/v3.8.36/PLANO-MELHORIA.md): 1. Quality Ratchet decoupled from flaky coverage (ci.yml): the shard→coverage→ratchet chain meant a single flaky Coverage Shard SKIPPED the whole Quality Ratchet on the release PR (v3.8.36 #4854), so cycle drift only surfaced post-merge in #5029. The job now runs on !cancelled(); coverage download is continue-on-error and the ratchet runs --allow-missing, so the DETERMINISTIC gates (eslint/complexity/cognitive/duplication/ codeql) stay blocking even when coverage is unavailable. 2. Fast-path drift gates (quality.yml PR→release): added check:complexity, check:cognitive-complexity, and a new lightweight check:pack-policy (pack-artifact unexpected-files check WITHOUT a build, via --policy-only) so drift + stray-tarball-file regressions are caught/rebaselined PER-PR instead of cascading onto the release PR. 3. Build heap default 4096→8192 MB (build-next-isolated.mjs): the clean graph peaks ~3.9 GB and brushed the old 4 GB ceiling; 8 GB gives headroom. Classify the specific path in LOCAL_ONLY_API_PREFIXES so loopback enforcement runs unconditionally before any auth check.
