* fix(thinking): recognize adaptive thinking + parse/scrub DSML tool-call markers
Two defects combined to break DeepSeek-V4-Flash turns and raise 502
empty_response on Claude Code autocompact.
Defect 1 — DSML tool-call markers leaked as visible content:
DeepSeek-V4-Flash occasionally emits tool calls in a non-standard DSML
text format using full-width pipes instead of the OpenAI tool_calls JSON.
Two shapes appear in production call logs:
- complete block: <|DSML|:Read><path>...</path></|DSML|:Read>
- stray closers (truncated call): </|DSML|parameter></|DSML|invoke>
</|DSML|tool_calls>, sometimes trailing a system-prompt echo
The openai-compatible path never parsed these, so the markers leaked to
the client as visible content and the turn ended incomplete.
Fix: add open-sse/utils/dsmlToolCalls.ts — parseDsmlToolCalls() converts
complete DSML blocks into OpenAI tool_calls and strips stray closing
markers from content (streaming-safe via a holdback for partial openers).
Wire it into the response translator before extractXmlInvokeBlocks so
DSML and XML invoke tool calls share the same pending queue.
Defect 2 — adaptive thinking silently suppressed:
A prior inline === 'enabled' check on body.thinking.type silently
suppressed adaptive (the intent Claude Code actually sends), so
reasoning was dropped. The model then emitted DSML tool-call markers
as plain text, producing an incomplete stop finish. Fix: use
hasActiveClaudeThinking() (which recognizes enabled AND adaptive) to
set requestedThinking, thread it through stream.ts and translator
state, and gate thinking block emission on state.requestedThinking
so upstream reasoning_content only relays when the client opted in.
Tests: 29/29 (6 dsml-tool-calls, 5 thinking-active-claude-adapter,
3 translator-resp-dsml-integration, 15 translator-resp-openai-to-claude
incl. requestedThinking suppression regression). typecheck:core clean.
* fix(sse): strip echoed system-prompt preamble + preserve large analysis/summary blocks
DeepSeek-V4 and similar models echo the OMNIROUTE_SYSTEM_INSTRUCTION_APPEND
directive (appended to the system tail by claude-to-openai.ts) and whole chunks
of the system prompt (<analysis>/<system-reminder>/<summary> blocks, prose
reproductions of the superpowers skill section) verbatim at the START of their
reply — the 'system message leak' persisting after the request-side fix.
Add two streaming-safe preamble strippers in directivePreambleStripper.ts:
- createDirectivePreambleStripper(directive): drops a leading reproduction of
the exact configured directive across arbitrary SSE chunk boundaries.
- createSystemPreambleStripper(): removes <analysis>/<system-reminder>/
<summary> echo blocks and known prose heads (Phase B) from the very start
of a stream, only while the stream is still a preamble.
Wire both into openai-to-claude.ts content-delta path: chain the exact-directive
stripper then the system-echo stripper before DSML/XML-invoke parsing, so a
leading system echo is dropped before it reaches the client.
Preserve large blocks (>= SYSTEM_ECHO_THRESHOLD=1000 chars) and blocks with no
trailing content — these are the model's real response (e.g. a Claude Code
autocompact summary), not a short system-echo. Stops the autocompact
empty-response regression where a whole-summary <analysis> block was stripped
to empty (3a8515).
Regression: origin's markdown-boundary feature (bufferedPrefix /
splitMarkdownBoundary, commit 1b39873ea) is preserved — preamble strip runs
before the markdown buffer rehydration, and the scrubbed content flows into
the existing DSML/XML-invoke/markdown pipeline unchanged.
TDD: tests/unit/directive-preamble-strip.test.ts (7 cases),
system-preamble-strip.test.ts (12 cases incl. 3a8515 regression),
system-preamble-wiring.test.ts (3 integration cases); group F regression
24/24 green; typecheck:core 0 errors.
* fix(sse): gate thinking block on requestedThinking + synthesize text block for reasoning-only responses
Reasoning-content (thinking) blocks were emitted unconditionally to
Claude-format clients, leaking reasoning to thinking-opt-out clients
(Claude Code sends thinking:{type:"disabled"}) — the operator reported
'reasoning is exposed'. On reasoning-only upstream responses (GLM-5.2
autocompact pattern), the unconditional thinking block also caused either
a 502 'no content block' at flush, or — after a text-block fallback — an
autocompact 'empty response' rejection that looped the session forever.
Streaming translator (openai-to-claude.ts):
- Compute hasReasoning outside the emission gate; accumulate into
state._reasoningAccum always (so fix B can fire).
- Gate only the thinking-block EMISSION on requestedThinking === true.
- FIX B at finish: when no text block was started and requestedThinking
!== true, synthesize a text content block from _reasoningAccum so
autocompact can extract the summary (no 502, compact applies).
- Skip fix B when requestedThinking === true to avoid double-exposure
(thinking block + text block both carrying reasoning).
Non-streaming translator (responseTranslator.ts):
- Thread requestedThinking through translateNonStreamingResponse into
convertOpenAINonStreamingToClaude.
- suppressThinking = requestedThinking === false: drop the thinking block
when content is present (no leak); relay reasoning as a text block when
the response is reasoning-only (no 502). requestedThinking === undefined
keeps the legacy 'always a thinking block' relay.
chatCore.ts: pass hasActiveClaudeThinking(body) to the non-stream
translate call (inline, since the shared const is in the stream branch's
temporal dead zone here).
Tests: 25/25 (5 gate-restore, 1 gate-502-repro, 4 nonstream-leak,
15 resp-openai-to-claude incl. requestedThinking suppression regression).
Group E (22) + F (14) regression-free. typecheck:core clean.
* fix(sse): restore requestToolIdentityMap in the Codex CLI responses-translation path
The needsResponsesTranslation branch (openai-responses -> openai, used
when the client also speaks Responses) silently dropped the
requestToolIdentityMap argument to createSSETransformStreamWithLogger
when the requestedThinking parameter was added, reverting the #7936
tool-identity round-trip fix for that branch. The sibling
needsTranslation branch was updated correctly; restore the same
argument here.

still called it with the pre-existing positional argument list, so the
new parameter silently absorbed the old customToolNames slot, and the
GLM_STREAM_BUFFER_BYTES tuning value (#12925) landed on
requestToolIdentityMap instead of streamBufferBytes — a TS2345 (number
is not assignable to Map<...> | null) caught by
check:open-sse-typecheck, and a real loss of GLM's 64KB stream buffer
budget. Insert an explicit `undefined` for requestedThinking to restore
the original alignment.

* fix(sse): make the system-preamble stripper opt-in and flush it at stream end
`createSystemPreambleStripper()` was wired DEFAULT-ON and unconditional in the
openai→claude streaming translator, unlike the exact-directive stripper right
above it, which only runs when the operator configured
OMNIROUTE_SYSTEM_INSTRUCTION_APPEND. Cause: the system-echo stripper recognises
its openers by English-prose heuristics ("# Skill usage", "# Verification
Process", <analysis>/<summary>/<system-reminder>), so leaving it always-on made
it mutate the response payload of EVERY openai→claude stream. A legitimate reply
opening with "# Skill usage: how to write one\n\nHere is the guide." lost that
whole section. It is now gated on OMNIROUTE_STRIP_SYSTEM_PREAMBLE=1, mirroring
the directive stripper's opt-in.
Second cause, same feature: neither stripper was ever flushed. Both buffer while
a construct is still undecided — a directive prefix that never completes, an
<analysis> block that never closes — and nothing released that buffer at the end
of the stream. A reply consisting of an unterminated echo block therefore reached
the client as an EMPTY message: the answer was held in the buffer and discarded
with the stripper. Both strippers now expose flush(), the finish handler calls it
for both, and the released text is emitted as a text block. A construct that WAS
finally classified as an echo is not resurrected (the drop is final).
Tests: tests/unit/system-preamble-gate-and-flush.test.ts pins the default-off
contract, the opted-in behaviour, the flush for both strippers (unit + wiring),
and the no-resurrection guard. system-preamble-wiring.test.ts now opts in
explicitly, since it exercises the stripping path.
* fix(sse): thread the client's thinking intent into the non-streaming path
The streaming and non-streaming translators disagreed on the default meaning of
`requestedThinking`, so the SAME request produced different shapes depending on
`stream`. Cause: chatCore computes the client's intent
(hasActiveClaudeThinking) and threads it into the SSE translator, which relays
reasoning as a thinking block only when it is explicitly `true` — but NO caller
ever passed it to translateNonStreamingResponse(). The non-streaming
OpenAI→Claude conversion therefore only ever saw `undefined`, its legacy
"always relay a thinking block" default, and leaked reasoning to a client that
had opted out with `thinking: {"type":"disabled"}`. The streaming plumbing also
coerced an omitted value into an explicit `false`, hiding the divergence behind
two different spellings of "no intent".
Fix (least destructive of the options): do NOT flip either gate — both encode a
deliberate, regression-tested contract — but give the non-streaming path the
same input the streaming path already has. runNonStreamingProviderLeg owns the
client body (`sourceBody`), so it computes the intent with the very same helper
and passes it down through translateNonStreamingClientResponse. `undefined`
keeps its documented back-compat relay for callers that cannot express intent
(issue-7856 / issue-6623), and stream.ts no longer defaults the parameter to
`false`, so "absent" now means the same thing in both signatures.
No content is lost by the suppression: a reasoning-ONLY response is still
relayed as an ordinary text block (no empty response, no 502) — exactly what the
streaming finish handler does.
Tests: tests/unit/nonstream-requested-thinking-parity.test.ts drives the real
provider leg with thinking disabled / enabled / adaptive.

Pollinations and Perplexity-web can answer a genuine failure (expired
session, exhausted free-tier credits) with HTTP 200 and a structurally
normal completion whose assistant text is just the provider's own
error prose. classifyProviderError() only inspects the body for
400/401/402/403/429, and detectMalformedNonStream() only checked
structural emptiness, so the error text reached the client as a real
answer and combo/auto-fallback never triggered.
Adds classifyFakeSuccessBody() in errorClassifier.ts — allowlisted to
pollinations/perplexity-web, reusing the existing
CREDITS_EXHAUSTED_SIGNALS/ACCOUNT_DEACTIVATED_SIGNALS phrase lists,
gated on short content with a dominant signal match — and wires it
into detectMalformedNonStream() so the existing malformed-200 /
combo-failover path picks it up with no other handler changes.
Regression test: tests/unit/diagnostics-fake-success-13461.test.ts
* fix(sse): bound active streams without terminal events
* fix(sse): derive the active-stream ceiling from the largest registered model budget
The watchdog is a hard lifetime cap that never resets on bytes, so a flat
15-minute default killed models the registry already allows to run for 20
minutes (the Codex entries declare timeoutMs: 1_200_000). The default is now
that maximum plus a one-minute margin, and a new test re-derives the maximum
from the registry so a future larger budget fails the gate instead of silently
re-opening the bug.

* fix(vision-bridge): extract/replace images nested inside tool_result content
Claude Code sends tool_result images as {type:"image",source:{base64}}
nested inside a tool_result's content array, not as top-level content
parts. The vision-bridge guardrail's extractImageParts filtered nested
hits out (!p.nested), so these images were silently dropped — a
text-only executor then received a request with no image and returned
HTTP 400.
Port the path-based nested extraction/replace fix:
- MediaPart gains a path field: the key/index chain from
message.content[partIndex] down to the media object itself.
- inspect() tracks the path through recursion; pushPart stamps it.
- extractImageParts drops the !p.nested gate and emits path for nested
hits (extract↔replace contract preserved: same order, every hit
replaceable).
- replaceImageParts rewrites via detectMediaParts: top-level hits swap
their content slot, nested hits walk MediaPart.path via the new
replaceObjectAtPath helper.
- ensureBase64ImagesForClaudeWire skips nested hits (.filter(!p.path))
to keep its sequential index map aligned.
TDD: 7 failing tests (path field, nested extract, nested replace,
document order) → 47/47 pass. typecheck:core clean.
* fix(vision-bridge): resolve provider prefix to node id for credential check
Re-land 932002580 (2026-08-19), which was never merged: it branched off
7acddd91a and fell outside the group-D reimplementation range (4f01fba68
re-picked only cf4dfc868). The same root cause now surfaces on the reroute
path (visionBridgeRerouteTextOnly=true): hasUsableCredentialsForModel
queried provider_connections with the bare node prefix "skhynix" → 0 rows
→ false → getBestVisionModel discarded the configured fixed model and
auto-selected cloudflare-playground/moonshotai/kimi-k2.7-code → Playwright
chromium missing → 502 on every image-bearing request.
- resolveProviderCredentialIds: literal prefix + prefix-index mapped node
id (no-op dedup), composed after #10760's alias→canonical
resolveProviderId.
- getPrefixToNode: 60s-cached getProviderPrefixIndex lookup, fail-open
null.
- hasUsableCredentialsForModel: loop the resolved provider ids and return
true when any has a usable active connection; noauth empty-set
semantics (#10702) preserved.
TDD: resolveProviderCredentialIds 4/4 + skhynix node-id integration test
(RED confirmed: false !== true on the reroute regression). Focused
regression green: visionBridgeCredentials 10/10, vision-bridge reroute/
credentials suite 12/12, vision-bridge policy/mode/cache 18/18,
visionBridgeRouter 16/16. typecheck:core clean.

A genuine 0-byte upstream empty response (GLM-5.2 on a huge autocompact
context returns ONLY reasoning_content or nothing, then closes) reaches
stream.ts::emitClaudeEmptyStreamErrorAndAbort which emits a 502 with
code "empty_response" via the onFailure callback AND propagates the
failure down the pipeline as controller.error(new Error(msg)). The plain
Error carries no .code, so getUpstreamErrorIdentifier (reads only
error.code) returns undefined, result.errorCode/result.errorType become
undefined, and the single-model retry guard (chat.ts) only matches
errorType === "stream_early_eof" / errorCode === "STREAM_EARLY_EOF".
The 502 surfaces to the client with no re-attempt (call logs
1788132529140-96ef4a / 1788142914004-062cf6, ~48s, tokens out=0).
This is the same class of transient upstream glitch STREAM_EARLY_EOF was
built for (HTTP 200 then zero useful frames — #3758), but empty_response
was never wired into the retry path.
Fix (three chokepoints, all required for consistency):
- stream.ts: emitClaudeEmptyStreamErrorAndAbort now propagates an Error
carrying code="empty_response" so a downstream classifier can identify
it (plain new Error(msg) dropped it).
- chatHelpers.ts: shouldRetryStreamEarlyEof now treats "empty_response"
the same as "STREAM_EARLY_EOF" via RETRYABLE_STREAM_EMPTY_CODES Set —
ONE bounded same-connection re-attempt, never a loop
(STREAM_EARLY_EOF_MAX_RETRIES=1 unchanged).
- chat.ts: the single-model retry guard now also enters on
errorCode === "empty_response".
The bounded retry never marks the account unavailable (an empty response
is a transient upstream glitch, not a bad key), mirroring #3758.
Tests: 5/5 (stream-empty-response-retry-96ef4a). Existing 3758 regression
guard stays green (5/5). typecheck:core clean.
* fix(sse): make direct response-start timeout reasoning-aware to stop 504 on high-effort TTFB
Reasoning models (GLM-5.2/5.3 reasoning.effort=high/max, codex-gpt-5.x-high,
third-party Claude-format replicas) warm up with a ~78s+ TTFB before
emitting the first byte. The stream-readiness layer (streamReadinessPolicy)
already budgets 180s for this class, but the fetch-layer guard
(resolveDirectHeadersTimeoutMs) was a flat 30s — it pre-empted a warm
reasoning response the readiness layer would have permitted, surfacing a
504 (regression introduced by 142ae9349).
Fix: resolveDirectHeadersTimeoutMs now accepts the request body and, when
hasHighReasoningEffort(body) matches a quoted "reasoning_effort" or nested
"effort" field with value high/max, raises the budget to
REASONING_READINESS_CEILING_MS (180_000) — aligning to the same ceiling the
readiness layer uses. The operator env override (OMNIROUTE_DIRECT_HEADERS
TIMEOUT_MS) is treated as a FLOOR: reasoning awareness only raises the
budget, never lowers it; an override above the ceiling (e.g. 240s) is
preserved.
proxyFetch.ts passes the request body (when it is a string) to
resolveDirectHeadersTimeoutMs so the budget is per-request.
The HIGH_REASONING_EFFORT_PATTERN is a bounded, non-overlapping regex
(no variable-length quantifier overlap) — no ReDoS surface (PII rule #1).
Tests: 7/7 (direct-response-start-timeout-reasoning-504 — flat default,
env override, high/max ceiling bump, floor semantics, non-reasoning
pass-through). typecheck:core clean.

The failure mode was concrete — a matched reasoning rule dropped on native Responses/Anthropic paths, model-suffix/account defaults, or fallback preparation, and `_omnirouteReasoningRule` leaking upstream as `Unsupported parameter` — and the fix is carried in the request-local credential context through dispatch, refreshed credentials and fallbacks, with forced effort winning over defaults and client-forged markers dropped at ingress. The 11-case integration suite exercises the real routing/translation modules.
Validated as a combined board first (this PR merged with the 4 siblings of the JxnLexn wave on the release tip): eslint with the frozen suppressions, typecheck:core, check:open-sse-typecheck, complexity, cognitive-complexity, changelog-integrity, docs-sync, migration-numbering, provider-consistency and a duplicate-identifier audit all green, plus 77 passing / 0 failing focused node:test cases across the test files the wave touches. **What the rework added on top of the contribution:** the new DB health-check behaviour is gated behind a default-off feature flag (`src/shared/constants/featureFlagDefinitions.ts`, `defaultValue: "false"`), documented in `docs/reference/FEATURE_FLAGS.md` with the description key carried into all 66 locales, so the release default is unchanged and the new bounds only apply when an operator opts in. The optional-FTS5 migration set was reconciled by hand with the "180" entry that landed meanwhile (`src/lib/db/migrationRunner/constants.ts`).
**Carried from your rebased head:** the `/api/db/health` local-only classification in `src/server/authz/routeGuard.ts` plus its `routeGuard` assertion — `runManagedDbHealthCheck()` forks native diagnostics into a child process, so Hard Rules #15/#17 apply. Re-verified here: 37 pass / 0 fail.

Closes a real process-killer: the direct-response start timeout could fire after the fetch promise had already settled, and aborting at that point delivered the abort reason to a promise nobody was awaiting — Node promotes that to an `unhandledRejection` → `uncaughtException` and the process dies (#12861). The timer is now a no-op once the attempt has settled, and the same guard is extended to combo hedge cancels and upstream fetch failures.

Groq is now excluded from Chrome TLS impersonation in both the direct and the proxied dispatch branch, even when `TLS_FINGERPRINT_PROVIDERS` is unset or explicitly lists it — Cloudflare answers the spoofed fingerprint with 1010 `browser_signature_banned` (#13225).
Scope is contained: the whole path is behind `ENABLE_TLS_FINGERPRINT`, which defaults to off, so nothing changes for operators who never opted in.

Two related fixes: the combo health probe sends `reasoning_effort: "none"` for Gemini-family models so the probe budget is not spent on thinking, and `detectMalformedNonStream` stops classifying a response with `finish_reason` `length`/`tool_calls`/`content_filter` and empty content as `empty_choices`.
The second half is the important one: it brings the post-translation check in line with `isEmptyContentResponse` (`open-sse/services/errorClassifier.ts`, `LEGIT_EMPTY_OPENAI_FINISH`), which already treated those finish reasons as legitimate. Until now a response could pass the pre-translation check and still be rewritten into a synthetic 502 afterwards — for every non-streaming completion, not just combo probes.

Behind the new `OPENCODE_RESPONSES_STALL_ROTATION` flag (default off): a streamed Responses reply with no first body byte within `RESPONSES_FIRST_BYTE_TIMEOUT_MS` (15s) cools the account and rotates once; a second stall fails fast instead of waiting the 80s readiness timeout.

`proxy_logs` records `upstream_status`, the HTTP status the provider actually returned through the proxy, instead of only success/timeout/error.

Behind the new `PROXY_SKIP_RECENTLY_FAILED` flag (default off), pool rotation and the opencode account rotation remember a proxy that just failed (refused probe or 429) and skip it for a doubling cooldown instead of re-serving it immediately.

Estimated token usage is now visible to operators: usage a provider marks as `estimated` carries an internal marker through extraction and the call log records `_omniroute.usageEstimated: true` on the logged response.

Behind the new `RETRY_AFTER_PROVENANCE_ENABLED` flag (default off): `unavailableResponse` omits the synthetic `Retry-After: 1` when there is no real retry signal, marks `retry_after_provenance` on its bodies, and both combo drain readers parse prose retry hints from plain-text bodies too. With the flag off, headers and bodies are exactly as before.

GHSA-r4q7-7f24-m29p. `CREDENTIAL_PATTERNS` (open-sse/utils/credentialPatterns.ts)
is the single catalog iterated in order by both the opt-in credential-masker
guardrail and the public error sanitizer. It had no entry for Groq (`gsk_`) or
xAI (`xai-`), and only knew the exact 48-char OpenAI `sk-` form.
Measured on the release tip before this change:

The public path already caught every `sk-` shape through STRONG_CREDENTIAL_TOKEN,
so the advisory's "both layers" framing only holds for gsk_/xai-; for the sk-
family the exposure was the guardrail.
Adds `groq` and `xai` after `anthropic_alt`, and a generic `openai_compatible`
`sk-` fallback as the LAST entry. Ordering matters: both consumers replace as
they iterate, so `openai_proj`, `openai` and `anthropic*` stamp their specific
label first and the fallback only sees shapes nothing else claimed. The
lookbehind mirrors STRONG_CREDENTIAL_TOKEN so `risk-…`-style words do not match.
All three regexes are a fixed prefix plus one bounded character class — linear,
no nested quantifiers.
Tests are red-first: the new guardrail cases (bare / sentence / JSON-body
contexts per shape, plus label-ordering and negative cases) and the catalog
coverage array in error-sensitive-redaction both failed on the tip.
Follow-ups deliberately left out of scope: `tskey-auth-` (Tailscale) was never in
the catalog, and the guardrail does not decode `\uXXXX` escapes the way the
public path does.
Proxy credentials containing a literal `%` no longer throw `URIError`: every `decodeURIComponent` on proxy user/password is guarded.

Fixes the production build break from `node:fs` reaching client bundles (`oauth.ts → cursorAgentCliVersion.ts` through the codebuddy-cn registry) and widens the client-bundle guard so it finds any Node builtin, not just the one that broke.

A 36-minute outage with a full host-level write-up is the best kind of bug report. The WAL at 2.98 GB being the high-water mark a VACUUM leaves behind — rewriting a 3 GB database after deleting ~125 rows, with auto-checkpoint never shrinking the file and TRUNCATE only every 6 hours — is exactly the kind of thing that is invisible from inside the process, where the V8 heap read 300 MB while RSS was dominated by 1.9 GB of glibc main-heap.

What changed since your version:
The tip had already cleared the TS2554 by deleting the 16th argument, leaving a comment that the highWaterMark stays at the helper default. So the base-red you found is gone, but the 64 KB #12179 asked for was still not applied and your new parameter had no caller. glm.ts now passes it, which is what turns the capability into the fix.
Your test file also hung the runner: every stream createSSEStream builds arms a 10s idle watchdog via setInterval in start, and nothing cancelled them, so node:test waited on a non-empty event loop long after the assertions passed. Cancelling each readable in an after hook runs the cancel handler that clears the timer — the file now reports in about 7 seconds. Worth knowing for future stream tests.
Your five assertions are unchanged and all pass. Reading the writable's desiredSize to measure the queue budget the stream was actually built with, rather than standing in for it, is the detail that makes this testable at all — and the 0-budget case pinning `??` against `||` is the kind of thing that silently rots otherwise.

O #12151 cobriu só metade: passthrough emitia o chunk final de usage, translate calculava a estimativa **depois** de fechar o stream, então o número só chegava ao log do servidor e nunca ao cliente. Fechar essa metade é o que faz a feature existir de fato.
Não emitir segundo chunk quando o upstream já mandou usage real é o detalhe que impede a correção de virar contagem dobrada.

`agent_message` chegando num fallback de Chat Completions é um item que o cliente não sabe interpretar; mapear ou descartar é a escolha certa, e escolher por item em vez de derrubar a resposta inteira mantém o fallback útil.
Vazamento de credencial em corpo de erro. `tests/unit/error-sanitizer-sk-key-qv45.test.ts` falhava no tip em 8ms:

O padrão era `/AIza[0-9A-Za-z_-]{35}/` — comprimento **exato**. Uma chave Google padrão tem 39 caracteres e casa; qualquer credencial `AIza…` mais curta ou mais longa passava direto para o corpo do erro.
Os dois lados divergiram na reconciliação de dois PRs do mesmo GHSA: o padrão com `{35}` veio do #12506, o teste anti-drift que cobra `/\\bAIza[A-Za-z0-9_-]{20,}/` veio do #12620. Está vermelho desde que os dois entraram em sequência.
`{20,}` no lugar de `{35}`. Numa mensagem de erro, redigir demais uma string que apenas começa com `AIza` não custa nada; redigir de menos vaza credencial — o lado errado para errar é claro.
Evidência: o arquivo vai de 7/9 para **9/9**. Bateria de sanitização com 538 testes: 533 passam, e as 5 restantes são pré-existentes no tip, não desta mudança (4 levam 21–25s por spawn de processo isolado sob carga; `tunnel-routes-error-sanitization` falha igual no tip puro, verificado). Nenhum teste foi enfraquecido — o padrão foi ampliado para satisfazer uma asserção que já existia.
Validado numa worktree combinada com as 16 PRs desta leva sobre `release/v3.8.51`: typecheck:core limpo, check-file-size e check-changelog-integrity OK, complexity 2788/3218 e cognitive 1261/1437, ESLint 0 erros nos 152 arquivos alterados, e a suíte vitest:ui completa (2149) verde.
Sobre esta PR especificamente: rodei os **23 arquivos de teste** que ela toca sobre o tip final, depois do merge da base — **392/392**. A migration `174_server_tool_executions.sql` não colide (o tip está em 173, e você já a renumerou em `c35f0fd7`).
O dono foi consultado antes do merge, porque o loop está atrás da flag `SERVER_OWNED_TOOL_LOOP_ENABLED` mas o primeiro send não-streaming mudou de dono sem flag, e a verificação manual em combo com Memory continuava desmarcada. A condição dele foi: entra se os testes focados passarem aqui. Passaram.
O lock de passthrough (`fetchCalls.length === 1`) é a parte que mais me convenceu — o double-dispatch que um `if (stream)` em volta do send existente causaria é exatamente o tipo de regressão que não aparece em teste de comportamento, só em contagem de chamada.
**Três ajustes meus na sua branch:**
1. `tests/unit/chatcore-stream-error-result.test.ts` procurava `"const legResult = await runNonStreamingProviderLeg"`, mas o seu commit final `6077b9dd` passou a reatribuir `legResult` e trocou para `let`. O guard falhava na sua própria branch (confirmei que o arquivo e o `chatCore.ts` eram byte-idênticos ao head da PR, então não era efeito da leva). Passou a aceitar `const|let` — a intenção do guard é o try/catch em volta da chamada, não a palavra-chave.
2. `tests/integration/skills-pipeline.test.ts` foi de 1156 para 1338 linhas e estourou o `testCap` de 1200. Segui o mesmo caminho que você já tinha tomado em `a1d2d20d` para os testes unitários: extraí os três casos do server-owned tool loop para `tests/integration/server-owned-tool-loop-pipeline.test.ts` (259 linhas), com instância própria do harness. O glob `tests/integration/*.test.ts` pega o arquivo novo sem registro adicional. 3/3 verdes isolados.
3. O arquivo novo herdou cinco `any` do original — que só passavam por estarem congelados no `eslint-suppressions.json` sob o nome antigo. Tipei como `Record<string, unknown>`. E `tests/unit/non-streaming-finalization.test.ts` tinha dois argumentos não usados em `trackPendingRequest`, agora prefixados com `_`.
Nada disso toca produção nem enfraquece asserção.
Validado em lote numa worktree combinada com os 3 PRs desta leva sobre o tip de `release/v3.8.51`: os três boardaram sem conflito, `typecheck:core` limpo e **22/22** nos arquivos de teste que trazem.
O crescimento de `src/sse/handlers/chat.ts` (2450 → 2454) é do #12641 e vai num PR de rebaseline próprio.

A única "falha" não é falha: `tests/unit/autoCombo/strict-zero-cost-filter.test.ts` é um teste em estilo Vitest que eu incluí por engano na invocação do runner nativo do Node — ele quebra no import (`@vitest/runner`), não numa asserção. Ao investigar, descobri que esse arquivo não roda em nenhum dos dois runners hoje (o glob do `test:unit` não lista `autoCombo` e o `include` do Vitest só pega `.tsx` nessa pasta); é um problema pré-existente do repositório, sem relação com esta leva, e vou registrá-lo separadamente.
O #12636 conflitava apenas na lista de testes do `@omniroute/opencode-plugin/package.json`, de forma aditiva: o tip já tinha `models-fetcher.test.ts` (do #12607, irmão desta mesma leva) e o #12636 acrescenta `telemetry.test.ts`. Fiz a união dos dois lados (25 arquivos contra 24 de cada) em vez de escolher um, o que teria removido um arquivo da suíte do plugin em silêncio.

Validado sobre o tip de `release/v3.8.51` depois de reconciliar com o #12620, que entrou primeiro nesta mesma sessão e ataca a mesma classe de problema por outra arquitetura.
**A colisão e como foi resolvida.** O #12620 consertou o GHSA-qv45-56jc-4wmj adicionando `RAW_CREDENTIAL_PATTERNS` a `error.ts` e importando-os em `upstreamErrorPassthrough.ts`. Este PR resolve o mesmo problema quebrando `error.ts` em `errorSanitization.ts` + `errorPathRedaction.ts`. Mantive a divisão em módulos deste PR, porque ao comparar os dois vocabulários o dele já era mais amplo: o `STRONG_CREDENTIAL_TOKEN` daqui cobre `sk-`/`sk_` **com lookbehind e uma variante para a forma embutida** (que pega `sk-proj-…`), mais Slack `xox-`, AWS `AKIA`/`ASIA`, `github_pat_`/`ghp_`/`glpat-` e JWT de três segmentos.
A única forma que o #12620 carregava e este conjunto não tinha era a chave do Google (`AIza…`) — adicionada aqui, com o mesmo quantificador limitado que os irmãos usam (AGENTS.md → PII §1, já que isso roda sobre corpos upstream não confiáveis).
**A verificação não foi por inspeção.** Rodei as suítes do próprio #12620 contra esta estrutura: **48/48** em `error-sanitizer-sk-key-qv45`, `bifrost-relay-response-leak-9m72`, `search-baseurl-client-override-3f8g` e `search-baseurl-ssrf-guard` — incluindo a asserção anti-drift daquela suíte, que é o oráculo certo aqui: *para todo corpo que a camada de passthrough recusa como vazante, o sanitizador de fallback não pode devolvê-lo intacto*. Ela passa, então a propriedade de segurança dos três GHSAs sobrevive à troca de arquitetura.
Os 21 arquivos de teste deste PR: **259/259**. `typecheck:core` limpo.
Validado após reconciliar com o #12465, que entrou primeiro e criou o mesmo arquivo novo `open-sse/utils/streamReadiness.ts` com desenho divergente de cancelamento.
Mantive a versão desta branch, que defere o release do lock para quando a leitura em voo termina e faz `reader.cancel()` fire-and-forget — assim uma promise de provider que nunca resolve não torna o cancelamento ilimitado. A escolha não foi por preferência: rodei as suítes dos **dois** PRs contra ela, 21/21 no readiness compartilhado e **22/22** incluindo o boundary do Perplexity do próprio #12465. typecheck:core limpo.
