#13832. A user reported that `nvidia` and `openrouter` — added after the
initial setup — always failed chat with `No active credentials for provider: X`,
while on the same instance and the same minute `/api/providers/{id}/test`
returned valid and `/sync-models` pulled 82 models.
Reproducing the resolution chain on the tip shows no defect in it: a connection
created exactly as `POST /api/providers` creates one resolves for every model
tried, and creation order is irrelevant — the query is `provider = ? AND
is_active = 1`, there is no boot-time registry and no migration that backfills
only older rows.
The three-line AUTH log the reporter pasted is reachable from exactly one place:
the pool arriving EMPTY at the key-policy filter. Every post-query skip produces
a different message ("all N accounts unavailable"). So the connections exist and
are active; the calling key's `allowed_connections` / quota scope removed them —
the shape you get from a key minted before those providers existed, which is
also why the older providers on that key keep working.
The real defect is that nothing ever said so. `/test` and `/sync-models` address
a connection by id and never consult the key's scope, so they cannot contradict
it, and the one log line that hinted at the filter became `debug` in #11937.
`getProviderCredentials` now counts the connections it had before applying the
key policy and, when that filter is what emptied the pool, returns
`{ blockedByKeyPolicy, blockedCount }` instead of a bare null. `handleNoCredentials`
turns it into a 403 naming the allowlist and the fix, alongside the existing
allRateLimited/allExpired branches. 403, not 401: the credential is valid, this
principal just may not use it.
Test is red-first in tests/unit/chat-helpers.test.ts (it asserts the status, the
count and that the message names the gate).
This does not close the report on its own — it makes the next occurrence
self-explanatory. The reporter still needs to confirm their key's
allowed_connections/allowed_quotas.

Behind `PROXY_SKIP_RECENTLY_FAILED` (from #13578): a provider 429 received through a pool member sets that member aside and a 2xx clears it, for opencode providers.

`proxy_logs` records `upstream_status`, the HTTP status the provider actually returned through the proxy, instead of only success/timeout/error.

Behind `STREAM_EARLY_EOF_SIBLING_FAILOVER_ENABLED` (default off): after the bounded same-connection retry is spent, a stream that closed early fails over exactly once to a sibling connection.

- The PR's own failover test was red on its head: the `/v1/chat/completions` route's early-stream keepalive dropped the `X-OmniRoute-Selected-Connection-Id` header on the first cold request. Tests now drive `handleChat()` directly; the assertion was kept.
- "One hop" was one hop per connection (a 3-connection pool made 4 dispatches); it is now a single sibling hop per request, and when the pool runs out the original `STREAM_EARLY_EOF` 502 is returned instead of a generic `bad_gateway`, so combo-level detection keeps working. The source-regex timeout test became a behavioral one; flag description in all 59 locales.

No-auth providers now honor a recorded model-only lockout before `getProviderCredentials` hands back the synthetic `noauth` connection (#13483). That early return skipped the per-connection status pass, so a `model_capacity` lockout was recorded but never enforced, and every request re-tried the locked model for a wasted upstream round-trip before failing over.
Maintainer additions: carried your `tests/unit/noauth-model-lockout.test.ts` from #13527. 3 of its 4 cases fail on the release tip without the fix and pass with it. Rebaselined `src/sse/services/auth.ts` 3542→3556 in `file-size-baseline.json` with a dated annotation.

The one-shot framing is what makes this safe: a pin is an explicit operator act, so taking the inactive row only for that request, with siblings out of the pool and a 60s per-connection storm gate, keeps the blast radius at one request. Dashboard deactivate staying off is the right carve-out.
Reconciled against the tip after the batch landed: the `chatHelpers.ts` import block conflicted with `buildExhaustionOptions` (#12975) and both imports were kept. 41/41 across the probe suites afterwards.

Right call not to reuse the combo loop's `fallbackCount`: it only increments after a leg fails or is skipped, so the second target would still report 0 at dispatch. Stamping the ordered index (or round-robin offset) at the gate is the only place the number is actually known. Reconciled and merged. This branch was stacked on #12941, which has since landed, so it read as 1484 additions across 16 files and CONFLICTING. I merged the current `release/v3.8.51` into it rather than rewriting your branch: `open-sse/executors/opencode.ts` conflicted in six places where your side was a strict superset of the squashed #12941, and the tip had touched that file through nothing but #12941, so your side was taken whole. The PR now reads as its real 12 files, +839/-25.
The change itself is right: a flapping upstream 5xx aborting the whole agentic chain is exactly the case where rotating to the next healthy account is safe, and keeping it a separate arm from the 400-empty branch matters because that one has to clone-read the body while this one never touches it. Threading `correlationId` through so interleaved requests stay attributable — and never fabricating one when absent — is the right discipline.
Both geo-block regression suites pass alongside the new ones (43/43 across the five opencode test files), which is what proves the conflict resolution preserved #12941's behaviour.
I also tightened the batch's file-size rebaseline here: `src/sse/handlers/chat.ts` needed no bump at all (it lands at 2452, under its existing 2458 freeze) and `open-sse/executors/base.ts` needed only your +2. Scoping the lock to the MODEL rather than the connection is the right layer for a `400 "Model is unavailable"` — a multi-day upstream outage on one model should not darken the account. Good discipline on the two allowlist-adjacent changes: gating the `markAccountUnavailable` branch on `ruleScope === "model"` AND `status === 400` leaves every other status on its existing path, and deliberately not widening `FULL_TEXT_RULE_PROVIDERS` keeps the #10880 egress-bucketed 429 classification intact. Reading the cooldown from the rule instead of a literal at the call site is what makes it self-healing.

Um `ALL_TARGETS_SKIPPED` 503 que não diz qual janela esgotou é opaco justamente no momento em que o operador mais precisa saber. Alinhar os rótulos de janela AUTH com os da API de uso fecha a outra metade: dois nomes para a mesma coisa fazem o dashboard e o erro parecerem discordar.

**Dois consertos meus na sua branch.**
1. `typecheck:core` falhava com `TS2345` em `comboAttemptLoop.ts` (linhas 130 e 416): o `QuotaSkipTarget` declarava `connectionId?: string`, mas o `ResolvedComboTarget` carrega `string | null` para alvo não-pinado. Alarguei para `string | null` no tipo de diagnóstico em vez de estreitar o call site — o módulo só **lê** o campo e a linha 29 já narrowa com `typeof === "string"`, então null não custa nada ali. Isso apareceu porque o `comboAttemptLoop` mudou de forma no #12746/#12811, mergeados nesta mesma campanha depois que você cortou a branch.
2. O `roundRobinCombo.ts` foi de 1198 para 1205 e cruzou o teto de 1200 para arquivo novo. Congelei com justificativa: o arquivo já nasceu em 1198 quando o #12811 o levantou de dentro do `combo.ts`, e os diagnósticos em si vivem no `quotaSkipDiagnostics.ts`, sob o cap. Registrei que a próxima extração natural é o corpo do attempt loop, mas que ele acabou de ser movido e deve assentar antes de ser cortado de novo.

`blockExtraUsage: false` significa "pode usar crédito extra", nunca "esconda a conta antes de despachar" — a conta saía da rota justamente quando o crédito extra existia para ser usado. Regressão de v3.8.50 vinda do #9057, com o sintoma mais enganoso possível: `attempted: 0`. A política já tinha admitido o combo e a checagem era refeita em cada membro interno.
Manter o filtro por prefixo de provider e o `disableNonPublicModels` intactos é o que impede o short-circuit de virar um buraco na allow-list.

`recordRejectedRequestUsage` runs on the path where the request never reached the guardrail chain — circuit-breaker-open and combo-exhausted rejections — so the video-bridge guardrail never got the chance to rewrite the transcript, and the raw cues went straight into `call_logs`. Routing the body through `redactVideoTranscriptFieldsForLog` at the persistence boundary is the right place: a no-op clone for non-video bodies, structured field substitution for video ones, and not bypassable by cue content.
The `requestBody == null ? requestBody : …` guard keeps the existing "no body available" case behaving exactly as before, which the neighbouring test still covers.
Validated on `release/v3.8.51`: `tests/unit/rejected-request-usage.test.ts` green, including the new case asserting the secret cue text does not survive into the persisted detail and that the field reads `[redacted-video-transcript]`. `typecheck:core` and `lint` clean.

Validado sobre o tip de `release/v3.8.51` depois de reconciliar com o #12620, que entrou primeiro nesta mesma sessão e ataca a mesma classe de problema por outra arquitetura.
**A colisão e como foi resolvida.** O #12620 consertou o GHSA-qv45-56jc-4wmj adicionando `RAW_CREDENTIAL_PATTERNS` a `error.ts` e importando-os em `upstreamErrorPassthrough.ts`. Este PR resolve o mesmo problema quebrando `error.ts` em `errorSanitization.ts` + `errorPathRedaction.ts`. Mantive a divisão em módulos deste PR, porque ao comparar os dois vocabulários o dele já era mais amplo: o `STRONG_CREDENTIAL_TOKEN` daqui cobre `sk-`/`sk_` **com lookbehind e uma variante para a forma embutida** (que pega `sk-proj-…`), mais Slack `xox-`, AWS `AKIA`/`ASIA`, `github_pat_`/`ghp_`/`glpat-` e JWT de três segmentos.
A única forma que o #12620 carregava e este conjunto não tinha era a chave do Google (`AIza…`) — adicionada aqui, com o mesmo quantificador limitado que os irmãos usam (AGENTS.md → PII §1, já que isso roda sobre corpos upstream não confiáveis).
**A verificação não foi por inspeção.** Rodei as suítes do próprio #12620 contra esta estrutura: **48/48** em `error-sanitizer-sk-key-qv45`, `bifrost-relay-response-leak-9m72`, `search-baseurl-client-override-3f8g` e `search-baseurl-ssrf-guard` — incluindo a asserção anti-drift daquela suíte, que é o oráculo certo aqui: *para todo corpo que a camada de passthrough recusa como vazante, o sanitizador de fallback não pode devolvê-lo intacto*. Ela passa, então a propriedade de segurança dos três GHSAs sobrevive à troca de arquitetura.
Os 21 arquivos de teste deste PR: **259/259**. `typecheck:core` limpo.

P1 of #12150: transcript text no longer reaches call logs or durable memory in the clear.
Reconciled on merge — the two persisted-requestBody assertions were failing, and the failure signature was misleading enough to be worth recording. They reported "expected: true, actual: false", which reads like the redaction not applying. It was not: pollForCallLog waited at most 120 tries x 20ms = 2.4s for the asynchronous SQLite write, then returned null, so assert.ok(row) failed before any redaction assertion ran. The observed durations were 5253ms and 4467ms against that 2.4s ceiling — a starved runner, not a leak. The control test failing alongside the positive one was the tell: a real redaction defect would break one direction, not both.
Replaced the fixed try count with a 30s wall-clock deadline: far past any healthy write, still bounded, and a fast machine still returns on the first pass. A privacy test should not depend on how busy the box is.
Verified: 4/4 three consecutive times under synthetic load, and 3/3 unloaded beforehand. Note the synthetic load reached ~7, below the ~38 where the original failure appeared, but the budget is now 12.5x larger and deadline-based rather than count-based.
CircuitBreaker.execute() treated every resolved promise as a success, but handleChatCore() reports most upstream failures by resolving with { success: false, status: 5xx }. On the chat path that spurious _onSuccess() decayed failureCount right before the call site's _onFailure() for the same attempt, so a provider answering 503s indefinitely stayed CLOSED at failureCount: 1 and kept receiving traffic — the breaker was structurally unable to open. Combo dispatches hit the same cancellation through the shared per-provider breaker.
execute() now takes an optional per-call classifyResult; without it the resolved-means-success contract every throw-based caller relies on is unchanged. executeChatWithBreaker() passes ignore and the chat path accounts for the outcome exactly once where the request context lives, so a combo success is no longer counted twice.

When a built-in provider's id or alias reserves the prefix of an existing OpenAI/Anthropic-compatible node — v3.8.50 added openference with alias of, shadowing nodes created earlier with prefix of — the runtime error `No active credentials for provider: openference` gave the operator nothing to act on. It now explains that the prefix routed to the built-in, names the shadowed node, and logs an AUTH warning. Precedence is unchanged and the lookup runs only on the credential-failure path when no connection was tried, so the hot routing path is byte-identical.

The least-used strategy ranked candidates by lastUsedAt alone, so after a 429 excluded the active account the replacement could be one that was merely oldest while still carrying its own backoff — it served a single request before the next one settled on a healthy account, the one-request detour with two cache misses reported on Codex. least-used now applies the backoffLevel tie-break the round-robin fallback branch already had, ahead of the existing never-used / oldest / priority order.

* fix(resilience): per-model 402 on a passthrough gateway no longer terminalizes the whole connection
402 variant of #3027. Passthrough/gateway providers that multiplex many
models behind one credential (kilo-gateway, ollama-cloud, etc.) can 402
on a single PAID model while free models on the same key remain
perfectly usable. Previously any 402 unconditionally set the connection
to a terminal `credits_exhausted` status, which is never auto-recovered
without an operator reset — taking out every remaining model on that
provider, amplified further inside combo routing (measured: one 402
removed 9 of 14 fallback targets in a real combo, dropping success rate
from 98.3% to 74.2% on a fixed load test per the issue report).
Root cause (matches the issue's own analysis):
1. resolveTerminalConnectionStatus() returned "credits_exhausted" for
ANY status === 402, with no per-model/passthrough check.
2. The generic per-model lockout gate (404/429/>=500) excluded 402.
3. The #3027 403-branch is gated on `!terminalStatus` — since (1) already
resolves a terminal status for any 402 before that branch runs, simply
adding 402 to its condition alone would not have fired.
Fix:
- resolveTerminalConnectionStatus() now takes isPerModelQuotaProvider and
skips the connection-wide terminal path for a bare `status === 402`
when true, letting it fall through to the per-model lockout branch
instead. An explicit result.creditsExhausted (a provider's own
classification, independent of HTTP status) is untouched and remains
unconditionally terminal.
- Extended the existing #3027 per-model lockout branch to also handle
402 (reason "credits" vs "forbidden" for 403), reusing the same
cooldown/lockout machinery and log format.
- Single-credential (non-passthrough) providers are unaffected:
isPerModelQuotaProvider is false there, so a 402 still terminalizes
the connection as before — that behavior is deliberate for prepaid
API keys (#5239 / #10616).
Also checked the issue's 4th root cause (terminal statuses never
auto-recovering) against the current codebase: connectionRecovery.ts
already has a 30-minute credits_exhausted reprobe
(isCreditsExhaustedReprobeCandidate) that the issue's report — filed
against v3.8.49 — didn't account for. The other two files it names
(rateLimit.ts's clearStaleCrashCooldowns, tokenHealthCheck.ts's
OAuth-refresh skip) legitimately exclude credits_exhausted for
unrelated reasons and are not bugs. Moot regardless: this fix prevents
credits_exhausted from being set at all for the passthrough case, so no
recovery wait is needed in the first place.
Tests: tests/unit/auth-passthrough-per-model-402-12242.test.ts, modeled
on the existing #3027 precedent test (real DB-backed integration test
via auth.markAccountUnavailable). Covers: paid-model-only lockout with
free model unaffected, a subsequent free-model request succeeding after
a sibling paid-model 402, single-credential 402 still fully terminal,
and no connection-wide backoff escalation on repeated 402s.
Verified:
- node --import tsx/esm --test tests/unit/auth-passthrough-per-model-402-12242.test.ts: 4/4 pass
- All related pre-existing tests (auth-ollama-cloud-per-model-403-3027,
auth-terminal-status, openrouter-free-model-credits-exhausted,
vertex-passthrough-model-lockout, 10347-embed-402-cooldown): 27/27
pass, no regressions
- npm run typecheck:core: 0 errors
- npm run check:cycles: no cycles
- eslint (auth.ts + new test file, with project suppressions): 0 errors

Local process execution failures (ENOENT spawn errors, binary missing, EPIPE, exit codes) were incorrectly treated as upstream provider failures, opening provider circuit breakers and cooling down valid connections. Added `isLocalExecutionError` guard to skip circuit breaker trips and connection disables when local host execution fails.
Mantém conexões forçadas indisponíveis com escopo correto, com teste próprio ampliado (`forced-connection-fallback.test.ts`). Validado no worktree combinado. Obrigado!
Remove a reserva estática global e passa a gatear o roteamento pela ocupação real do lease exclusivo ativo, com boa cobertura de testes (5 arquivos, 54 casos, todos verdes no worktree combinado). Typecheck limpo.
Dois ajustes feitos por cima antes do merge:
1. **26 arquivos `.pyc` órfãos removidos** (`scripts/ops/__pycache__/…`, `tests/unit/ops/__pycache__/…`) — cache compilado do Python sem relação com o fix de lease, provavelmente commitado sem querer do ambiente local.
2. **Conflito em `src/app/api/keys/[id]/route.ts`**: mantida a checagem mais ampla desta PR (`instanceof ApiKeyPolicyInvariantError || código LEASE_KEY_POLICY_INVALID`), que é um superset da versão anterior — cobre o caso original e o novo caminho de erro do lease.

One real conflict in open-sse/services/combo.ts at both LKGP-clear call sites (handleComboChat + round-robin path): the release tip already has #12013's clearStaleLKGP() helper, which this PR's branch predates — kept the current helper call at both sites, discarding the pre-refactor inline pattern. typecheck:core and the open-sse test suite (vitest, 9/9 on volumeDetector) both green after resync. Thanks for the well-scoped Phase 4 migration.
The v3.8.50 close left 134 post-freeze commits on release/v3.8.50 that never
reached the cycle branch (the freeze cut release/v3.8.51 at 3192eb88d5). A
plain merge of main reproduces all of them through the `Release v3.8.50`
squash against a July merge-base and conflicted on 551 files; merging the
release tip first, against the recent common ancestor, narrows the real
conflicts to 102 (51 generated, 51 judged file by file with a proof each —
see _tasks/postmortems/2026-08-25-release-v3.8.50-pipeline-eficiencia.md,
Parte IV). Step 2 brings main's own post-tag fixes and the finalized
CHANGELOG through scripts/release/sync-next-cycle.mjs.
Resolution rules applied, in order of evidence:
- generated files regenerated with the repo's own generators
(sync-llm-mirrors, gen-budget-card-svg, gen-provider-reference);
- where release/v3.8.51 already carried the same fix in a newer shape
(#11524 search sweep, #11551 catalog scheduler, Google BYOP retry, KIE
Market id map, Docker worker budget measured in #7518) its version stays;
- where release/v3.8.50 carried the newer shape (Volcengine cookie-domain
CodeQL fix + shared Zod schemas, #11355/#10534 cooldown release helper,
positive-anchor tests for security-hardening and cli-oneproxy) it wins;
- GPL-retired Raycast/Hailuo (#11691) stay retired: nothing of theirs comes
back and the public-route test keeps the retired route out;
- the ten changelog.d fragments of v3.8.50 are dropped — they are already
aggregated in main's CHANGELOG and would double-aggregate at v3.8.51.
Three things git's auto-merge silently produced were caught by a per-line
detector and fixed: providerLimits.ts lost T's imports and the
windowStillExhaustedAfterRealReset helper; catalogCache.ts and
providerLimits.ts kept both sides' identical copies of three declarations;
contextHandoff.ts's new provider-allowlist skip returned undefined against
the #11552 outcome type. Every decision was re-run through the tests both
sides own for it.
Preserves whether a 429 retry hint came from transport headers, structured google.rpc.RetryInfo, or unverified response-body text, and caps body-derived cooldowns at the operator's configured maxCooldownMs so an unverified upstream hint can no longer force an arbitrarily long model/semaphore lockout — authoritative header/structured resets stay intact across combo, chat, and Responses paths. Closes#11695. 29/29 focused tests passing. Thanks!
Hardens session-affinity key extraction: no more JSON.stringify on arbitrary request objects,
recognizes bounded text from Responses/chat/Anthropic/Gemini/common string-root shapes, enforces
a shared 4096-char processing budget, and rejects oversized explicit session IDs before
trim/regex/hash work. 128/128 focused affinity/failover tests passing. Closes#11744. Thanks!

with author preserved (auto-merged onto the just-boarded #11888); 10/10 focused tests +

Rebased onto the current release/v3.8.51 tip as part of a combined provider-retirement/provenance merge batch (Designer Web, Felo Web, Runtime, GPL-derived removal, Qwen Web already landed). Large conflict set (this is the biggest PR in the batch — the common ChatGPT Web provider touches chat, images, count-tokens, session leases, and combos). Conflicts resolved:
- `open-sse/config/providers/registry/chatgpt-web/*`, `open-sse/executors/chatgpt-web*`, `open-sse/handlers/imageGeneration/providers/chatgptWeb.ts`, and their tests: kept deleted, matching the PR's stated scope.
- `open-sse/config/providers/registry/minimax/web/index.ts`, `open-sse/handlers/imageGeneration/providers/geminiWeb.ts`, `open-sse/executors/gemini-web.ts`'s stale image-mode branch: base-drift collisions against already-merged sibling retirements (#11691, #11708) — kept deleted / dropped the dead code, since this PR's own branch forked before those merged.
- `src/shared/constants/reservedProviderPrefixes.ts`, `open-sse/executors/index.ts`, `executorProxy.ts`, `virtualFactory.ts`, `autoStrategy.ts`, `src/lib/db/providers.ts`, `src/sse/handlers/chat.ts`: combined the Designer + Runtime (Felo/Qwen) + common-ChatGPT-Web retirement guard calls at each shared chokepoint — compute-once-then-OR pattern, consistent with prior combinations in this batch.

- `tests/unit/chatcore-executor-proxy.test.ts`, `provider-node-reserved-prefix.test.ts`, `combo-auto-candidate-expansion.test.ts`, `messages-count-tokens-route.test.ts`, `virtual-auto-combo.test.ts`: split into independent per-mechanism test blocks (established pattern); `virtual-auto-combo.test.ts`'s old "includes cookie web-session providers" positive-inclusion test (which used chatgpt-web as its example) was retired along with the provider and replaced by this PR's negative-exclusion test for the same slot.
- `docs/architecture/ARCHITECTURE.md`, `CODEBASE_DOCUMENTATION.md` (+ 4 i18n mirrors), `README.md`, `FREE-TIERS-GUIDE.md`, `docs/diagrams/free-tier-budget.svg`, `docs/screenshots/free-tier-budget-card.svg`, `docs/reference/PROVIDER_REFERENCE.md`: recomputed every stale count from the real merged state — 104 executors (`countFiles` gate logic), 351 providers (regenerated via `gen:provider-reference`), 152/351 `hasFree` entries, 445/438/7 free-tier catalog rows, 13 ToS-avoid providers, budget-card regenerated via its real generator script. One doc conflict (`oauth/` module list) needed picking HEAD's side specifically — theirs still listed the already-removed `raycast` module instead of the real `openference`.
- `config/quality/test-masking-allowlist.json`: additive merge of the PR's 17 `_deletedWithReplacement` entries alongside the batch's existing ones (one real duplicate-key mistake in my first pass, caught and fixed via a `object_pairs_hook` duplicate-key check before finalizing).
Also fixed two real, unrelated-to-my-merge issues surfaced by the focused suite:
- `tests/unit/resolve-web-provider-host.test.ts`: the PR's own test had a typo — it asserted `perplexity-web`'s resolved host as `"perplexity.ai"`, but the provider's registered `website` is `"https://www.perplexity.ai"` and the resolver returns the URL's `host` verbatim (no www-stripping), so the correct value is `"www.perplexity.ai"` (consistent with the same test's own `url` assertion).
- `tests/unit/hard-session-lease-bypass-inventory.test.ts`: this golden call-site inventory was already stale on the pristine post-#11713 tip (confirmed via a throwaway probe worktree) — `src/lib/db/providers.ts`'s 3 connection-fallback sites and a third `src/app/api/providers/route.ts` site were never added to the golden list by the earlier-merged #11698/#11720 PRs. Updated it to the real current inventory (dated inline comments explain each delta and which PR introduced it), plus this PR's own legitimate deltas (image-edits duplicate-call removal, `ChatGptWebExecutor.execute()` site removed).
Focused suite green (433/433 across executor-proxy, reserved-prefix, hard-session-lease-bypass-inventory, resolve-web-provider-host, retirement/runtime-block/source-retirement/management-retirement/image-handler-retirement, migration-168, combo-auto-candidate-expansion, virtual-auto-combo, executor-map-golden and siblings), plus `typecheck:core`, `check-file-size`, and `check-changelog-integrity` clean.
