9bc7eb8fd2 fix(vision-bridge): nested tool_result images + provider-prefix credential check (#12903)
* fix(vision-bridge): extract/replace images nested inside tool_result content

Claude Code sends tool_result images as {type:"image",source:{base64}}
nested inside a tool_result's content array, not as top-level content
parts. The vision-bridge guardrail's extractImageParts filtered nested
hits out (!p.nested), so these images were silently dropped — a
text-only executor then received a request with no image and returned
HTTP 400.

Port the path-based nested extraction/replace fix:
- MediaPart gains a path field: the key/index chain from
message.content[partIndex] down to the media object itself.
- inspect() tracks the path through recursion; pushPart stamps it.
- extractImageParts drops the !p.nested gate and emits path for nested
hits (extract↔replace contract preserved: same order, every hit
  replaceable).
- replaceImageParts rewrites via detectMediaParts: top-level hits swap
their content slot, nested hits walk MediaPart.path via the new
  replaceObjectAtPath helper.
- ensureBase64ImagesForClaudeWire skips nested hits (.filter(!p.path))
to keep its sequential index map aligned.

TDD: 7 failing tests (path field, nested extract, nested replace,
document order) → 47/47 pass. typecheck:core clean.

* fix(vision-bridge): resolve provider prefix to node id for credential check

Re-land 932002580 (2026-08-19), which was never merged: it branched off
7acddd91a and fell outside the group-D reimplementation range (4f01fba68
re-picked only cf4dfc868). The same root cause now surfaces on the reroute
path (visionBridgeRerouteTextOnly=true): hasUsableCredentialsForModel
queried provider_connections with the bare node prefix "skhynix" → 0 rows
→ false → getBestVisionModel discarded the configured fixed model and
auto-selected cloudflare-playground/moonshotai/kimi-k2.7-code → Playwright
chromium missing → 502 on every image-bearing request.

- resolveProviderCredentialIds: literal prefix + prefix-index mapped node
  id (no-op dedup), composed after #10760's alias→canonical
  resolveProviderId.
- getPrefixToNode: 60s-cached getProviderPrefixIndex lookup, fail-open
  null.
- hasUsableCredentialsForModel: loop the resolved provider ids and return
true when any has a usable active connection; noauth empty-set
  semantics (#10702) preserved.

TDD: resolveProviderCredentialIds 4/4 + skhynix node-id integration test
(RED confirmed: false !== true on the reroute regression). Focused
regression green: visionBridgeCredentials 10/10, vision-bridge reroute/
credentials suite 12/12, vision-bridge policy/mode/cache 18/18,
visionBridgeRouter 16/16. typecheck:core clean.

d70f43d4b4 fix(sse): retry empty_response 502 + reasoning-aware direct response-start timeout (#12906)
* fix(sse): retry 0-byte empty_response 502 like STREAM_EARLY_EOF to stop autocompact 502

A genuine 0-byte upstream empty response (GLM-5.2 on a huge autocompact
context returns ONLY reasoning_content or nothing, then closes) reaches
stream.ts::emitClaudeEmptyStreamErrorAndAbort which emits a 502 with
code "empty_response" via the onFailure callback AND propagates the
failure down the pipeline as controller.error(new Error(msg)). The plain
Error carries no .code, so getUpstreamErrorIdentifier (reads only
error.code) returns undefined, result.errorCode/result.errorType become
undefined, and the single-model retry guard (chat.ts) only matches
errorType === "stream_early_eof" / errorCode === "STREAM_EARLY_EOF".
The 502 surfaces to the client with no re-attempt (call logs
1788132529140-96ef4a / 1788142914004-062cf6, ~48s, tokens out=0).

This is the same class of transient upstream glitch STREAM_EARLY_EOF was
built for (HTTP 200 then zero useful frames — #3758), but empty_response
was never wired into the retry path.

Fix (three chokepoints, all required for consistency):
- stream.ts: emitClaudeEmptyStreamErrorAndAbort now propagates an Error
carrying code="empty_response" so a downstream classifier can identify
it (plain new Error(msg) dropped it).
- chatHelpers.ts: shouldRetryStreamEarlyEof now treats "empty_response"
the same as "STREAM_EARLY_EOF" via RETRYABLE_STREAM_EMPTY_CODES Set —
  ONE bounded same-connection re-attempt, never a loop
  (STREAM_EARLY_EOF_MAX_RETRIES=1 unchanged).
- chat.ts: the single-model retry guard now also enters on
  errorCode === "empty_response".

The bounded retry never marks the account unavailable (an empty response
is a transient upstream glitch, not a bad key), mirroring #3758.

Tests: 5/5 (stream-empty-response-retry-96ef4a). Existing 3758 regression
guard stays green (5/5). typecheck:core clean.

* fix(sse): make direct response-start timeout reasoning-aware to stop 504 on high-effort TTFB

Reasoning models (GLM-5.2/5.3 reasoning.effort=high/max, codex-gpt-5.x-high,
third-party Claude-format replicas) warm up with a ~78s+ TTFB before
emitting the first byte. The stream-readiness layer (streamReadinessPolicy)
already budgets 180s for this class, but the fetch-layer guard
(resolveDirectHeadersTimeoutMs) was a flat 30s — it pre-empted a warm
reasoning response the readiness layer would have permitted, surfacing a
504 (regression introduced by 142ae9349).

Fix: resolveDirectHeadersTimeoutMs now accepts the request body and, when
hasHighReasoningEffort(body) matches a quoted "reasoning_effort" or nested
"effort" field with value high/max, raises the budget to
REASONING_READINESS_CEILING_MS (180_000) — aligning to the same ceiling the
readiness layer uses. The operator env override (OMNIROUTE_DIRECT_HEADERS
TIMEOUT_MS) is treated as a FLOOR: reasoning awareness only raises the
budget, never lowers it; an override above the ceiling (e.g. 240s) is
preserved.

proxyFetch.ts passes the request body (when it is a string) to
resolveDirectHeadersTimeoutMs so the budget is per-request.

The HIGH_REASONING_EFFORT_PATTERN is a bounded, non-overlapping regex
(no variable-length quantifier overlap) — no ReDoS surface (PII rule #1).

Tests: 7/7 (direct-response-start-timeout-reasoning-504 — flat default,
env override, high/max ceiling bump, floor semantics, non-reasoning
pass-through). typecheck:core clean.

* docs(changelog): add fragments for empty_response 502 retry + reasoning-aware timeout

f1e7148c19 fix(routing): preserve reasoning overrides across transports and fallbacks (#13556)
Merged. The failure mode was concrete — a matched reasoning rule dropped on native Responses/Anthropic paths, model-suffix/account defaults, or fallback preparation, and `_omnirouteReasoningRule` leaking upstream as `Unsupported parameter` — and the fix is carried in the request-local credential context through dispatch, refreshed credentials and fallbacks, with forced effort winning over defaults and client-forged markers dropped at ingress. The 11-case integration suite exercises the real routing/translation modules.

Validated as a combined board first (this PR merged with the 4 siblings of the JxnLexn wave on the release tip): eslint with the frozen suppressions, typecheck:core, check:open-sse-typecheck, complexity, cognitive-complexity, changelog-integrity, docs-sync, migration-numbering, provider-consistency and a duplicate-identifier audit all green, plus 77 passing / 0 failing focused node:test cases across the test files the wave touches. The wave's i18n fill (new keys carried to all 66 locales), free-tier doc counts and file-size rebaseline land in one follow-up PR right after the wave, as with #13904.

Thank you — and for keeping this a runtime-only change with the editor and service-tier work in their own PRs.

c4293e28e2 fix(cursor): recover Kimi tool calls emitted as history narration (#12723)
Merged. Kimi emitting tool calls as history narration is a provider quirk we have to absorb rather than pass through; recovering them keeps the tool contract intact for clients that never see the quirk.

Validated as a combined board first (this PR merged with the 21 siblings of the same wave on the release tip): eslint with the frozen suppressions, typecheck:core, check:open-sse-typecheck, complexity, cognitive-complexity, changelog-integrity, i18n new-key coverage, docs-counts, docs-sync, migration-numbering, provider-consistency and a duplicate-identifier audit all green, plus 176 passing / 0 failing focused node:test cases across the 25 test files the wave touches and the dashboard test under Vitest (2/0). Then re-validated alone on the fresh tip before this merge: conflicts re-resolved, file sizes rebaselined for this PR's own growth, eslint and this PR's focused tests re-run.

Thank you.

e7999c477b fix(db): bound health scans and isolate native diagnostics (#13717)
Merged after a maintainer rework that kept every one of @HouMinXi's commits intact.

**What the rework added on top of the contribution:** the new DB health-check behaviour is gated behind a default-off feature flag (`src/shared/constants/featureFlagDefinitions.ts`, `defaultValue: "false"`), documented in `docs/reference/FEATURE_FLAGS.md` with the description key carried into all 66 locales, so the release default is unchanged and the new bounds only apply when an operator opts in. The optional-FTS5 migration set was reconciled by hand with the "180" entry that landed meanwhile (`src/lib/db/migrationRunner/constants.ts`).

**Carried from your rebased head:** the `/api/db/health` local-only classification in `src/server/authz/routeGuard.ts` plus its `routeGuard` assertion — `runManagedDbHealthCheck()` forks native diagnostics into a child process, so Hard Rules #15/#17 apply. Re-verified here: 37 pass / 0 fail.

Thank you for the depth of this one — the resource-bounds suite and the sql.js startup/backup coverage are the kind of tests that keep a database layer honest.

f36fcd8c31 fix(resilience): extend process crash guard to combo hedge cancels and upstream fetch failures (#13636)
Closes a real process-killer: the direct-response start timeout could fire after the fetch promise had already settled, and aborting at that point delivered the abort reason to a promise nobody was awaiting — Node promotes that to an `unhandledRejection` → `uncaughtException` and the process dies (#12861). The timer is now a no-op once the attempt has settled, and the same guard is extended to combo hedge cancels and upstream fetch failures.

4902ac8128 fix(network): skip Chrome TLS impersonation for Groq (#13445)
Groq is now excluded from Chrome TLS impersonation in both the direct and the proxied dispatch branch, even when `TLS_FINGERPRINT_PROVIDERS` is unset or explicitly lists it — Cloudflare answers the spoofed fingerprint with 1010 `browser_signature_banned` (#13225).

Scope is contained: the whole path is behind `ENABLE_TLS_FINGERPRINT`, which defaults to off, so nothing changes for operators who never opted in.

ce98c30cfb fix(combos): stop Gemini thinking from failing dashboard combo tests (#13560)
Two related fixes: the combo health probe sends `reasoning_effort: "none"` for Gemini-family models so the probe budget is not spent on thinking, and `detectMalformedNonStream` stops classifying a response with `finish_reason` `length`/`tool_calls`/`content_filter` and empty content as `empty_choices`.

The second half is the important one: it brings the post-translation check in line with `isEmptyContentResponse` (`open-sse/services/errorClassifier.ts`, `LEGIT_EMPTY_OPENAI_FINISH`), which already treated those finish reasons as legitimate. Until now a response could pass the pre-translation check and still be rewritten into a synthetic 502 afterwards — for every non-streaming completion, not just combo probes.

d8c0448293 fix(opencode): rotate once when a Responses stream stalls before its first byte (#13484)
Behind the new `OPENCODE_RESPONSES_STALL_ROTATION` flag (default off): a streamed Responses reply with no first body byte within `RESPONSES_FIRST_BYTE_TIMEOUT_MS` (15s) cools the account and rotates once; a second stall fails fast instead of waiting the 80s readiness timeout.

Maintainer rework before merge (kept the idea, no default behavior change):
- The TLS first-byte watchdog from #12656 is restored byte for byte (the PR had changed its pump, timer and cancel); the stall guard lives in its own module.
- Proxy-less multi-account setups now rotate the same way as proxied ones (the original threw for them), a client abort during the wait rethrows instead of rotating, and the env var is documented as flag-only.

Validated first on the combined board of all 38 PRs of this batch (10 merged as-is, 28 after the maintainer rework) on top of release/v3.8.51 c0f92ec: typecheck:core, check:open-sse-typecheck and check:dashboard-typecheck clean; ESLint clean on every changed file; file-size (rebaselined for the combined growth), complexity, cognitive-complexity, changelog-integrity, docs-counts, docs-sync, migration-numbering and i18n new-key gates green; 735 focused node:test cases with the only batch-caused failure (a flag-count assertion) fixed. 238cb1b076 feat(proxy-logs): keep the HTTP status the provider actually returned (#13580)
`proxy_logs` records `upstream_status`, the HTTP status the provider actually returned through the proxy, instead of only success/timeout/error.

Maintainer rework before merge (kept the idea, no default behavior change):
- The migration collided with the tip (177 was already taken): renumbered to `179_proxy_logs_upstream_status.sql`, the runner's already-applied check moved to `case "179"` (the old `"177"` would have skipped the tip's own 177), migration count bumped to 176 in README, AGENTS.md, llm.txt and its mirrors (operator-approved).
- A new test runs the real migration runner on the real SQL files and fails with the old case number.

29d66cbf8c feat(proxies): stop re-serving a proxy that just failed (#13578)
Behind the new `PROXY_SKIP_RECENTLY_FAILED` flag (default off), pool rotation and the opencode account rotation remember a proxy that just failed (refused probe or 429) and skip it for a doubling cooldown instead of re-serving it immediately.

Maintainer rework before merge (kept the idea, no default behavior change):
- The original was on by default and re-queried the DB on every request while a member was set aside; selection now caches a refusal sequence number and re-runs the cascade once per set-aside event.
- `src/lib/db` no longer imports the heavy dispatcher for key normalization (a parity test guarantees the same key as `proxyConfigToUrl()`); `.env.example` and `ENVIRONMENT.md` document the default as false.

08fe9e5117 fix(usage): skip billing for estimated token usage, bill real output only on partial estimates (#13686)
Estimated token usage is now visible to operators: usage a provider marks as `estimated` carries an internal marker through extraction and the call log records `_omniroute.usageEstimated: true` on the logged response.

Maintainer rework before merge (kept the idea, no default behavior change):
- Billing is unchanged: the original skipped cost/budget/quota-share for estimated usage, which would have let streams without upstream usage and eight web executors spend $0 against API-key budgets; that part is reverted and no opt-in flag was added because it cannot be made budget-safe.
- Both open-sse TS2345 errors, the client-visible `estimated_prompt_tokens` field and the `as unknown as` casts are gone; four real `handleChatCore` cases assert the marker and unchanged spend.

ac52d4d9ea fix(sse): omit synthetic Retry-After and mark retry provenance on drain path (#13672)
Behind the new `RETRY_AFTER_PROVENANCE_ENABLED` flag (default off): `unavailableResponse` omits the synthetic `Retry-After: 1` when there is no real retry signal, marks `retry_after_provenance` on its bodies, and both combo drain readers parse prose retry hints from plain-text bodies too. With the flag off, headers and bodies are exactly as before.

Maintainer rework before merge (kept the idea, no default behavior change):
- A past `Retry-After` date is no longer labelled as an upstream signal with `Retry-After: 1`; non-JSON bodies (HTML 502 pages) log at debug instead of warning on every request.
- The provenance claim is narrowed to responses built by `unavailableResponse`, documented in the flag row.

Validated first on the combined board of all 38 PRs of this batch (10 merged as-is, 28 after the maintainer rework) on top of release/v3.8.51 c0f92ec: typecheck:core, check:open-sse-typecheck and check:dashboard-typecheck clean; ESLint clean on every changed file; file-size (rebaselined for the combined growth), complexity, cognitive-complexity, changelog-integrity, docs-counts, docs-sync, migration-numbering and i18n new-key gates green; 735 focused node:test cases with the only batch-caused failure (a flag-count assertion) fixed. e498c349e3 fix(security): add Groq, xAI and OpenAI-compatible key shapes to the credential catalog (#13744)
GHSA-r4q7-7f24-m29p. `CREDENTIAL_PATTERNS` (open-sse/utils/credentialPatterns.ts)
is the single catalog iterated in order by both the opt-in credential-masker
guardrail and the public error sanitizer. It had no entry for Groq (`gsk_`) or
xAI (`xai-`), and only knew the exact 48-char OpenAI `sk-` form.

Measured on the release tip before this change:

| Shape | public sanitizer | guardrail |
|------------------------------|------------------|-----------|
| Groq gsk_ + 52 | LEAK | LEAK |
| xAI xai- + 80 | LEAK | LEAK |
| DeepSeek sk- + 32 hex | redacted | LEAK |
| sk- + 20/36/40/51 (not 48) | redacted | LEAK |

The public path already caught every `sk-` shape through STRONG_CREDENTIAL_TOKEN,
so the advisory's "both layers" framing only holds for gsk_/xai-; for the sk-
family the exposure was the guardrail.

Adds `groq` and `xai` after `anthropic_alt`, and a generic `openai_compatible`
`sk-` fallback as the LAST entry. Ordering matters: both consumers replace as
they iterate, so `openai_proj`, `openai` and `anthropic*` stamp their specific
label first and the fallback only sees shapes nothing else claimed. The
lookbehind mirrors STRONG_CREDENTIAL_TOKEN so `risk-…`-style words do not match.
All three regexes are a fixed prefix plus one bounded character class — linear,
no nested quantifiers.

Tests are red-first: the new guardrail cases (bare / sentence / JSON-body
contexts per shape, plus label-ordering and negative cases) and the catalog
coverage array in error-sensitive-redaction both failed on the tip.

Follow-ups deliberately left out of scope: `tskey-auth-` (Tailscale) was never in
the catalog, and the guardrail does not decode `\uXXXX` escapes the way the
public path does.

215ac43a70 fix(proxy): keep proxy credentials holding a literal percent (#13605)
Proxy credentials containing a literal `%` no longer throw `URIError`: every `decodeURIComponent` on proxy user/password is guarded.

Maintainer rework before merge (kept the idea, no default behavior change):
- HTTP proxies still failed because undici's `ProxyAgent` decodes the credentials itself; the dispatcher now builds undici's `Basic` token with the safe decoder and passes it as `token`, so a literal `%` works there too.
- The three remaining unguarded sites (`mappers.ts`, `proxySubscription/parse.ts`, `subscriptionService.ts`) are guarded; tests run the real `createProxyDispatcher` against a local HTTP CONNECT proxy and a local SOCKS5 server that record what they received.

Then re-validated alone on the fresh release tip right before this merge: ESLint on the changed files, typecheck:core, check:open-sse-typecheck, the file-size/complexity/changelog gates and this PR's own tests.

1d17c239d1 fix(build): stop the client bundle from reaching server-only modules, and make the guard find them (#13436)
Fixes the production build break from `node:fs` reaching client bundles (`oauth.ts → cursorAgentCliVersion.ts` through the codebuddy-cn registry) and widens the client-bundle guard so it finds any Node builtin, not just the one that broke.

Maintainer rework before merge (kept the idea, no default behavior change):
- The guard was 11× slower (3.8s → ~40s) because resolved edges were not cached; with resolved edges and per-file verdicts cached it runs in ~4.6s.
- Bare builtins that Next's client build polyfills (`path`, `os`, `crypto`, `buffer`, … from Next's own `resolve.fallback` list) are allowed consistently; `node:` imports are always flagged; a drift test fails if Next stops polyfilling an allowlisted name.

Then re-validated alone on the fresh release tip right before this merge: ESLint on the changed files, typecheck:core, check:open-sse-typecheck, the file-size/complexity/changelog gates and this PR's own tests.

8bad85f58e fix(logging): mask *-api-key headers in request-log pipeline (#13273) (#13401)
Masks every `*-api-key` header spelling (`x-goog-api-key`, `api-key`, `xi-api-key`) in the request-log pipeline by matching on the hyphen-compacted key, and adds `xi-api-key` to the payload redaction set (#13273). The new test drives the real `createRequestLogger` / `protectPayloadForLog` paths.

Validated in one consolidated batch of this series (37 PRs boarded together on `release/v3.8.51`): `typecheck:core`, `check:open-sse-typecheck` and `check:dashboard-typecheck` clean; ESLint clean on every changed file; file-size, complexity, cognitive-complexity, changelog-integrity, docs-counts, docs-sync and migration-numbering gates green (only the pre-existing `open-sse/utils/stream.ts` file-size red remains, inherited from the base); 3,743 focused `node:test` cases plus 34 vitest cases green.

01a66e26c6 fix(ci): clear two base-reds on release/v3.8.51 — mutation-coverage gate + image-only-model guard (#12945)
Merged as part of the owner batch of 2026-09-11.

This PR had a live worktree in another session, so it sat outside the main 39. Merged on your explicit call, validated first rather than taken on trust: boarded with the other 10 worktree-held PRs into a consolidated worktree off `release/v3.8.51`.

- ESLint over every changed file: no errors
- `typecheck:core` clean; `check:dashboard-typecheck` OK; `check:changelog-integrity` OK
- complexity 2821 / baseline 3218 and cognitive-complexity 1272 / baseline 1437
- 203 of 208 assertions green. The 5 remaining (`guide-settings-route` ×4, `hard-session-lease-bypass-inventory` ×1) reproduce on the pure tip with nothing from this batch applied.
- `imageGeneration.ts` rebaselined 3259 → 3293 for #12945's image-only-model guard, landed separately in #13392 so nothing was pushed onto a live branch.

⚠️ base-red inherited: #12732 — provider count 356 vs 358 and `open-sse/utils/stream.ts` 3115 > frozen 3098, both reproducing on the pure tip.

2cb02d26c6 fix(sse): exempt tiny-budget reasoning probes and trace persisted-cooldown skips (#12659) (#13269)
Merged as part of the 39-PR owner batch of 2026-09-11, validated as a unit.

Boarded into one consolidated worktree cut from `release/v3.8.51` with the other 38 — zero conflicts between them.

- ESLint over every changed file: no errors (the only finding was one suppression entry the batch emptied, pruned on #13243)
- `typecheck:core` clean; `check:dashboard-typecheck` OK (206 pre-existing, within baseline); `check:changelog-integrity` OK
- complexity 2821 / baseline 3218 and cognitive-complexity 1272 / baseline 1437 — both under baseline
- 256 assertions green: 246 under node:test and 10 under vitest, which is where `tests/unit/**/*.test.tsx` actually runs
- `check-file-size`: `chatCore.ts` rebaselined 6144 → 6146 for #13278 and #13276, annotated and landed on #13243

⚠️ base-red inherited: #12732 — the provider count (356 in the docs vs the 358 the modules define) and `open-sse/utils/stream.ts` at 3115 > frozen 3098 both reproduce on the pure tip with zero contribution from this batch.

359b9b520b fix(db): self-heal under sustained memory pressure, VACUUM gate, WAL housekeeping (#13011)
A 36-minute outage with a full host-level write-up is the best kind of bug report. The WAL at 2.98 GB being the high-water mark a VACUUM leaves behind — rewriting a 3 GB database after deleting ~125 rows, with auto-checkpoint never shrinking the file and TRUNCATE only every 6 hours — is exactly the kind of thing that is invisible from inside the process, where the V8 heap read 300 MB while RSS was dominated by 1.9 GB of glibc main-heap.

Reconciled against the tip after the batch landed: the `ENVIRONMENT.md` table conflicted with #13035's `OMNIROUTE_SQLJS_WASM_PATH` row and both sides were kept.

---

Validated in one consolidated worktree cut from `release/v3.8.51`, boarded with the other 19 PRs of this batch. Two in-batch conflicts, both additive and resolved by keeping each side: the `ENVIRONMENT.md` table (#13035 + #13011) and the `chatHelpers.ts` import block (#12975 on the tip + #13017).

- `typecheck:core` clean; `check:dashboard-typecheck` OK (206 pre-existing, within baseline); `check:changelog-integrity` OK; `check:docs-counts` migrations ✓
- complexity 2816 / baseline 3218 and cognitive-complexity 1271 / baseline 1437 — both under baseline
- 531 of 532 focused assertions green across the batch's 46 test files
- `check-file-size` rebaselined for the batch's real growth (annotation `_rebaseline_2026_09_11_mergebatch_v3851_houminxi`, landed on #13038), attributed per PR

The single red is **not this batch**: `tests/unit/combo/quota-weighted-strategy.test.ts` → "A/B isolation: 7 hard-empty + 2 at 0.5% + 1 at 40%, floor=1" asserts an order between two connections of identical weight and flakes on the pure tip too — 2 failures in 4 runs at `origin/release/v3.8.51` with nothing from this batch applied.

⚠️ base-red inherited: #12732 — `Docs Gates`, `Merge integrity`, `No new ESLint warnings`, `Unit Tests fast-path` and `Fast Quality Gates` reproduce on the pure tip (provider count 356 vs the 358 the modules define, SKILL.md drift, and `open-sse/utils/stream.ts` at 3115 > frozen 3098, untouched here).

Thanks @HouMinXi — the live evidence on these (X500 logs, `storage.sqlite` state, real `/v1/models` probes, the 36-minute outage write-up) is what let a 20-PR batch be reviewed as a unit.

af49d4972e fix(stream): accept the buffer size glm.ts has been passing since #12179 (#12925)
Rebased onto the tip and completed, per the maintainer's call to finish the wiring rather than merge the capability alone.

What changed since your version:

The tip had already cleared the TS2554 by deleting the 16th argument, leaving a comment that the highWaterMark stays at the helper default. So the base-red you found is gone, but the 64 KB #12179 asked for was still not applied and your new parameter had no caller. glm.ts now passes it, which is what turns the capability into the fix.

Your test file also hung the runner: every stream createSSEStream builds arms a 10s idle watchdog via setInterval in start, and nothing cancelled them, so node:test waited on a non-empty event loop long after the assertions passed. Cancelling each readable in an after hook runs the cancel handler that clears the timer — the file now reports in about 7 seconds. Worth knowing for future stream tests.

Your five assertions are unchanged and all pass. Reading the writable's desiredSize to measure the queue budget the stream was actually built with, rather than standing in for it, is the detail that makes this testable at all — and the 0-budget case pinning `??` against `||` is the kind of thing that silently rots otherwise.

Thank you also for separating your own red checks from the base's and reporting what you found there. That is how #12919's identical failures got explained instead of chased.

1216cff05b fix(streaming): fail fast when an upstream stream produces only lifecycle/heartbeat events, never real content (#12741)

91990d606b fix(sse): emit trailing usage estimate in translate streams when upstream stays silent (#12828)

89c42d36df fix(security): redact AIza credentials of any length in error bodies (#12964)
Vazamento de credencial em corpo de erro. `tests/unit/error-sanitizer-sk-key-qv45.test.ts` falhava no tip em 8ms:

```
AssertionError: Google key survived: Bad credentials for AIzaSyA1B2C3D4E5F6G7H8I9J0KaLbMcNdOeP
```

d6f315018a fix(chat): continue after a server-owned tool on Chat Completions (#12867)

3d2bcc9f12 feat(api): emit gateway-measured tokens-per-second excluding TTFT (#12631)

Obrigado, @RaviTharuma.

bb8e75a00d fix(resilience): surface Responses failed.error.message in 502s (#12472)

2265ce761f fix(security): harden public error boundaries (#12506)

`typecheck:core` limpo.

350ac8c12d fix(sse): preserve 1min.ai partial output before stream errors (#12466)

627fcba605 fix(sse): preserve Perplexity stream failures (#12465)

4a37c7f46e fix(security): close 3 advisories — search baseUrl exfil, sk- in the error sanitizer, bifrost relay header leak (#12620)

8729909622 fix(logging): raise the SSE payload collector's default cap (#12461)

Obrigado, @hartmark.

e2e330a058 perf(stream): compile hot-path regexes once, bound token caches, fix quadratic buffering (#12179)

- **`executeAttempt.ts`** — mantido o comentário explicativo do tip.

Também corrigi **quatro erros de typecheck que o branch introduzia**: `hasUnsupportedSignal` estava tipado `boolean` mas avaliava para `string | boolean`, e o fast-path de `extractUsage()` indexava `c.response`/`c.message` como `unknown`.

Obrigado, @opensource-elearning.
