feat(quota): pool budget = per-account limit × account count (summed balde)
A pool with N same-type connections now has an effective budget of perAccountLimit × N per dimension. Only the limit fed to fair-share scales — consumption (pool-keyed shared bucket) is unchanged.

*

* Task 5 — Summed budget (× connection count)
*
* Proves that a pool with N same-type connections exposes an effective budget of
* planLimit × N per dimension, so that consumption between L and 2L is ALLOWED
* for a 2-connection pool (whereas it would be BLOCKED with a single-connection pool).
*
* Strategy:
* - The account-count scaling happens inside enforceQuotaShare, which calls
* decideFairShare with the scaled limit. Because the real enforceQuotaShare
* requires a live DB (listAllocationsForApiKey, getPool, etc.), we test the
* scaling at two levels:
*
* Level A (unit — decideFairShare): feed decideFairShare directly with
* limit = planLimit × 1 → consumption > L → BLOCK (single-account baseline)
* limit = planLimit × 2 → same consumption → ALLOW (2-account summed budget)
* This mirrors exactly what enforce.ts injects after the accountCount multiply.
*
* Level B (snapshot shape): build the dimensionsInfo array as enforce.ts does and
* assert that `dimensionsInfo[0].limit === 2 * planLimit` for a 2-connection pool.
*
* Both levels are deterministic and need no live DB or Redis.
