chore(quality): rebaseline tokenHealthCheck.ts for the Claude soft-fail refresh fix

src/lib/tokenHealthCheck.ts grows from 1214 to 1221 lines, past the frozen
ceiling of 1218 (which had only 4 lines of headroom). The growth is the fix
itself: preserving the refresh_token and telling a transient failure apart
from a dead credential needs extra state on the sweep path, which cannot
leave the module without breaking its internal API.

Covered by tests/unit/tokenHealthCheck-claude-refresh-token-preserved.test.ts;
the 14 sibling tokenHealthCheck/credentialHealth test files were run together
on the combined tree (72/72).
