* fix(sse): stop the auto-combo candidates inspector from dropping blocked rows (#9133) prepareVirtualAutoComboInputs applied filterResilienceBlockedCandidates before the #7819 read-only candidate inspector ever saw the pool, so a model-locked or cooled-down candidate silently disappeared from /auto-combo/*/candidates instead of showing up as reachable:false with a reason (modelLocked/connectionCooldown/breakerState were dead fields by construction). Add an opt-in `skip` parameter so the inspector builds its own unfiltered pool; routing (createVirtualAutoCombo/createBuiltinAutoCombo called without a prepared override) is unchanged. Also aligns isModelLocked's model argument to the bare model id, matching every lock writer and the routing-side filter, instead of the "provider/model" string. fix(combo): lock GitHub models rejected as "not supported" for future requests (#11781)
Follow-up to #11762/#11774, same bug class in combo's own model-lockout wiring: GitHub rejects several models (gpt-5.4, gpt-5.3-codex, etc.) with a 400 that's permanently unavailable for this account's Copilot integration, but nothing recorded a cross-request lockout — combo's #5249 in-request advance guard is correct but doesn't persist, so the same doomed model gets retried from scratch on every new request, indefinitely. Fix: on a model-scoped 400 (`isModelScoped400`), call `lockModelIfPerModelQuota(provider, connectionId, rawModel, "model_capacity", 1h)`. GitHub already has per-model-quota enabled, so only the rejected model locks — siblings keep working. `isModelLocked()` is already checked pre-dispatch, so no other wiring needed. Validated: 3/3 new tests + fixed a pre-existing test-isolation gap in combo-model-scoped-400-advance.test.ts (shared model name across sub-tests without clearing lockout state). Thanks! |

fix(resilience): stop hammering permanently-moved endpoints and billing-suspended accounts (#11774)
Follow-up to #11762, same bug class hitting freeaiapikey (410 permanently-moved endpoint) and fireworks (412 billing-suspension) — both fell through checkFallbackError's generic transient-cooldown branch and got retried every ~1 minute for a full day. Fix: `ENDPOINT_PERMANENTLY_MOVED_PATTERNS`/`isEndpointPermanentlyMoved()` → 24h lockout; `ACCOUNT_SUSPENDED_BILLING_PATTERNS`/`isAccountSuspendedForBilling()` → treated as credits-exhausted (1h cooldown), independent of status code so it also catches Fireworks' 412. #11762 landed first and touched the same file — rebased/re-merged onto the updated tip (additive, no logic changes) and re-validated: 13/13 tests pass. Thanks for tracing this with real production logs again! |

fix(resilience): lock permanently retired models instead of short backoff (Gemini ban prevention) (#11762)
Root-caused via a real Gemini-ban incident log: deprecated-model 404/410s (e.g. gemini-2.5-flash "no longer available to new users") fell through checkFallbackError's generic transient-cooldown branch, so combo/auto-routing kept re-selecting a permanently dead model every cooldown window forever — the hammering that got the account flagged as abusive. Fix: `MODEL_PERMANENTLY_UNAVAILABLE_PATTERNS` + `isModelPermanentlyUnavailable()` classify these as a 24h lockout instead, surfaced via `quotaResetHintMs` so combo's per-request model-lockout honors it in full. Validated: 6/6 new tests + 133/133 existing accountFallback/error-classification tests, no regressions. Thanks for tracing this end-to-end with real production logs! |

fix(providers): retire Qwen Web pending provenance review (#11713)
Rebased onto the current release/v3.8.51 tip as part of a combined provider-retirement/provenance merge batch (Designer Web, Felo Web, Runtime, GPL-derived removal all landed together already). Conflicts resolved: - `src/shared/constants/providerRetirement.ts`: add/add conflict — combined `felo-web`/`felo` (already-merged) with `qwen-web`/`qw` into one `RUNTIME_RETIRED_PROVIDER_IDS` set, kept both `assertRuntimeProviderAvailable`/`assertRuntimeModelProviderAvailable` helpers. - `open-sse/config/providers/registry/minimax/web/index.ts`: modify/delete — kept deleted (file is hailuo-web's registry entry, already retired by #11691; this PR's own change to it was just a comment reword on a since-removed target). - `open-sse/executors/index.ts`, `executorProxy.ts`, `virtualFactory.ts`, `autoStrategy.ts`, `model.ts`, `chat.ts`, `chatHelpers.ts`, `auth.ts`, `src/lib/db/providers.ts`, `reservedProviderPrefixes.ts`: combined the Designer + Runtime (Felo + Qwen) retirement guard calls at each shared chokepoint — compute-once-then-OR pattern, consistent with the prior Designer+Felo combination. - `src/shared/constants/providers/web-cookie.ts`, `tests/snapshots/provider/translate-path.json`, `tests/snapshots/executors/executor-map.json`: both sides had inserted a different retired provider (qwen-web vs. already-retired raycast/hailuo-web) at the same dict position — resolved by dropping both. `executor-map.json`'s `keyCount` recomputed to 135 (matches actual merged `entries`). - `tests/unit/chatcore-executor-proxy.test.ts`, `tests/unit/provider-node-reserved-prefix.test.ts`: split into independent Felo/Qwen test blocks (established pattern for coexisting retirement-mechanism tests); recomputed `RESERVED_PREFIX_COUNT` to 398 (Designer+Felo+Qwen tombstones on top of the post-#11691 REGISTRY, verified via direct module evaluation, not hand-derived). - `config/quality/test-masking-allowlist.json`: additive merge of Qwen's `_deletedWithReplacement` entries alongside Designer's. - `README.md` + all `docs/i18n/*/README.md` mirrors, `docs/getting-started/FREE-TIERS-GUIDE.md`, `docs/reference/FREE_TIERS.md`, `docs/diagrams/free-tier-budget.svg`, `docs/screenshots/free-tier-budget-card.svg`: recomputed the free-tier catalog counts (447 entries / 440 active / 7 discontinued) from the actual merged `freeModelCatalog.data.ts`, regenerated the budget-card SVG via its real generator (`scripts/research/gen-budget-card-svg.mjs`), and dropped the retired Qwen quick-start row / QWEN MODELS section from every i18n README (identical unlocalized block across all 34 locales). - Also fixed a duplicate-import merge artifact in `src/lib/db/providers.ts` (`isRuntimeRetiredProviderId` imported twice) caught by `typecheck:core`, and rebaselined `file-size-baseline.json` for the combined retirement-guard growth (`virtualFactory.ts` +3, with justification). Focused suite green (345/345 across executor-proxy, reserved-prefix, migration-167, qwen-web-retirement, virtual-auto-combo, web-cookie/session, executor-map-golden and siblings), plus `typecheck:core` and `check-file-size`/`check-changelog-integrity` clean. `Fast Quality Gates` has been failing on every open PR against release/v3.8.50 with "2 gate(s) failed: mutation-test-coverage lockfile". Neither belongs to any feature branch, so they are drained here. check:lockfile — a transitive dev/optional entry (libxmljs2 → brace-expansion@2.1.4) landed with a `resolved` URL pointing at registry.npmmirror.com instead of registry.npmjs.org, which lockfile-lint rejects as a supply-chain policy violation. Verified before touching it: the recorded `integrity` (sha512-hGfVzPxthbf3+2yjg/RBs60cB0FhqBS/zvdV/4wn4/BmN0bNMMHPc4V/BbFieqf1TKAGGAHnY4eSjajCl0f2Xg==) is byte-identical to the official npmjs tarball's, so the package content is the same and this is a provenance slip — someone's install ran behind the mirror registry — not a tampered package. Repointed the URL; `integrity` untouched. It was the only non-npmjs host in the lockfile (2690 npmjs entries). check:mutation-test-coverage — two covering unit tests were missing from stryker.conf.json's tap.testFiles, so their mutant kills did not count: repro-glm-iso-reset-24h-cap (accountFallback.ts) and repro-combo-persisted-cooldown-preskip (comboPredicates.ts). Inserted in place. Both gates verified green locally. The diff is three lines: re-serializing either file would have reordered a curated list for no reason. feat(search): first-class X Search via SuperGrok x_search (#10988)
⭐5 — Provider x-search de primeira classe (SuperGrok/xAI x_search) em POST /v1/search e MCP omniroute_x_search. Fallback de credenciais xai-oauth→xao→xai; distinto de web search e do X Developer MCP. Reconciliado com o release tip (que já incluía #10981 "skip catalog-default SearXNG" deste mesmo lote): merge trouxe 5 conflitos reais de contagem gerada (llm.txt/README.md/AGENTS.md/PROVIDER_REFERENCE.md/SVGs/46 mirrors i18n, todos verificados como bump puro 347→348, sem perda de conteúdo do HEAD) + 1 conflito real de mergeable=CONFLICTING. Durante a validação, os 3 testes novos de SearXNG expuseram um bug real de interação com #10981: `isUnconfiguredLoopbackSearchProvider()` checava o baseUrl ESTÁTICO do catálogo em vez do baseUrl efetivo (após override de `provider_options.baseUrl` ou `providerSpecificData.baseUrl` da conexão), então QUALQUER request a searxng-search — mesmo com override customizado — era rejeitado como se fosse o default não-configurado. Corrigido em `open-sse/handlers/search.ts` (resolve o baseUrl efetivo via `resolveSearchBaseUrl()` antes do skip-check, tanto para o provider primário quanto o alternate). Um teste do próprio #10988 que assumia o comportamento pré-#10981 (default localhost:8888 sempre atendido) foi atualizado para refletir o comportamento já mesclado e intencional (503 quando não configurado). Validação completa: typecheck limpo, 70/70 testes unit (search-route/search-registry/x-search-provider/searxng-loopback-default), 24/24 vitest MCP, 14/14 integration (search-providers-catalog), lint limpo nos arquivos tocados, docs-counts-sync OK (2 drifts soft pré-existentes, não relacionados), gates estáticos (file-size/complexity/cognitive/dead-code/changelog) todos OK. |

Database settings page returns HTTP 500 when SQLite lacks the optional dbstat table (#10558)
* fix(db): tolerate a SQLite build without the dbstat virtual table
getDatabaseStats() queried `dbstat` once per table with no guard. `dbstat` is
compile-time optional (ENABLE_DBSTAT_VTAB) and is absent from sql.js/WASM
builds, so on those runtimes the query throws and the error propagates out of
getDatabaseStats().
Every caller dies with it. Most visibly, GET and PATCH /api/settings/database
return HTTP 500, which makes the entire database settings page unusable — users
cannot read or change page size, cache size, or vacuum settings.
The function already anticipated missing virtual-table modules: the COUNT(*)
lookup a few lines above swallows "no such module:" errors. The dbstat query
simply sat outside that guard.
Probe dbstat once per call and skip the per-table size lookups when it is
unavailable, reporting size 0. Database-level figures (total size, page count,
cache size) come from pragmas and stay accurate; only per-table byte sizes are
lost, which is the correct trade against a hard 500.
Unrelated failures (I/O errors, corruption) still propagate.
Both spellings are handled: sql.js reports "no such module: dbstat" while
better-sqlite3 can surface "no such table: dbstat".
* test(db): cover prefixed driver errors and dbstat edge cases
Review follow-up on the previous commit.
The guard is deliberately unanchored because real drivers stringify errors
with their class name attached ("SqliteError: no such table: dbstat",
"RuntimeError: ..."). Nothing pinned that, so anchoring the regex would have
passed the suite while silently breaking every real driver. Add a case for the
prefixed form; it fails if a caret is introduced.
Also cover three shapes the fake previously could not express:
- a database with no user tables, which is what a fresh install hits first
- SUM(pgsize) returning NULL for a table occupying no pages
- dbstat answering the probe but failing on a later table, which documents
that a mid-iteration fault still propagates rather than being mistaken for
an absent module
Correct the source comment: the two error spellings track the SQLite build,
not the driver package, so the earlier attribution to better-sqlite3 was
wrong.
* docs(changelog): add fragment for the dbstat availability guard
Registers the new test with Stryker alongside the sibling db suites and adds
the changelog fragment for this fix.

Makes the ProviderErrorRule `scope` field real at the persistence layer, exclusively for agentrouter (owner decision; every other provider keeps byte-identical behavior). checkFallbackError now surfaces `ruleScope` behind the HONORS_RULE_LOCK_SCOPE_PROVIDERS allowlist, and the agentrouter 403 path consults the rules before the generic apikey-FORBIDDEN early-return. markAccountUnavailable honors scope "connection" with a temporary connection cooldown instead of a per-model lockout — guarded so a permanent state can never be downgraded to a transient retry loop — and combo now skips the exhausted account within the same request, which also stops force-reusing the just-cooled connection via allowRateLimitedConnection. agentrouter.org signals temporary quota exhaustion with HTTP 403/400 and a Chinese body (用户额度不足) instead of 429, so clients like Claude Code treat it as permanent and abort, and the fallback engine classified it as a generic apikey AUTH_ERROR. New registry open-sse/config/upstreamStatusRestatement.ts restates those statuses to 429 with a synthetic Retry-After at a single hook in chatCore's providerFailure block (after parseUpstreamError), so classification, combo aggregation and the client response all see a retryable error. 无权访问模型 (permanently no model access) is veto-listed and never restated. agentrouter classification rules are registered in providerErrorRules.ts and reach the real checkFallbackError path through resolveRuleMatchBody() with an exclusive FULL_TEXT_RULE_PROVIDERS allowlist — every other provider keeps its previous behavior byte-for-byte. fix(sse): stop the executor-contract guard from hot-looping the router (#10373)
The `instanceof Response` guard from #10256 broke two ways: 1. `instanceof` is nominal against `globalThis.Response`, but proxyFetch dispatches through the npm undici package's fetch, whose Response is a different class — so valid upstream responses were rejected as contract violations. Replaced with `isResponseLike()` (instanceof fast path + structural brand/member probe); genuinely malformed shapes still throw. 2. The thrown error had no `.status`, so it fell through to chatCore's BAD_GATEWAY default — an internal defect was treated as a flaky provider, cooling the connection down and retrying forever. It now carries status 500 + `executor_contract_violation`, registered as request-scoped and terminal (no cooldown, no breaker, no retry). batch_api.test.ts went from exit 124 (infinite hang, pinning Unit shard 4/4 in every open PR) to exit 0, 22/22 passing. fix(sse): make Claude effort/no-think catalog variants dispatchable on every provider (#9006)
* fix(executors): route Claude-via-Vertex through native rawPredict with real streaming
Claude models on Vertex AI were being sent through the generic OpenAI-
compatible partner endpoint, which 404s/errors for Claude on at least
some projects. Route them through Vertex's native Anthropic Messages
API (publishers/anthropic/.../rawPredict) instead, stripping the
body-level model field rawPredict rejects and injecting the required
anthropic_version field.
rawPredict only ever returns a complete JSON body, never real SSE
framing, so streaming requests now get a genuine Anthropic-format SSE
stream synthesized from that JSON (message_start/content_block_*/
message_delta/message_stop), which the existing claude-to-openai
response translator already knows how to parse.
Also fixes two response-format resolution bugs that silently dropped
a custom model's DB-stored targetFormat override whenever the model
id also existed in the static provider registry (as claude-sonnet-4-6
and claude-opus-4-7 do under vertex): resolveModelOrError had its own
ad-hoc resolution that never consulted the override, and even once
fixed, executeChatWithBreaker discarded the correctly-resolved format
before handleChatCore's own resolution ran a second time.

* refactor(cursor): extracts token extraction into shared lib
Moves tryIdeAuth/tryAgentAuth and supporting helpers out of the
auto-import route into src/lib/cursor/tokenExtractor.ts, and adds
an agent-cli-state.json fallback candidate path to tryAgentAuth
(alongside the existing auth.json candidate) so the extraction
logic can be reused by the upcoming renewal orchestrator.
* feat(cursor): adds cursor-agent-backed token renewal orchestrator
Builds the renewal orchestrator in src/lib/cursor/renewal.ts: a
bounded, unattended-safe --list-models nudge, a side-effect-free
status availability check, an in-flight spawn lock keyed by
command, and renewCursorConnection() which nudges cursor-agent
then independently re-scrapes the IDE and cursor-agent credential
sources to detect whichever refreshed. Extends cursorAgent.ts's
binary resolution and spawn helper with fixed-paths-only mode and
a SIGKILL follow-up for background use. Adds a generic keyed-mutex
utility (src/shared/utils/keyedMutex.ts) for serializing a
connection's renew-then-persist cycle, and forwards a busy-timeout
through driverFactory's node:sqlite fallback path.

manual-refresh route to exhaustive switch statements over the
renewal result. Adds a short-lived host-keyed dedup cache around
tryIdeAuth() so multiple due Cursor connections sharing a host
don't each open the same state.vscdb file in one sweep tick.
Adds opportunistic eviction to the manual-refresh cooldown map,
an outer try/catch to the availability route for defense-in-depth
consistency with the plan's other routes, and corrects a stale
JSDoc claim about the /login route's auth check. Documents the
now-empirically-confirmed agent-cli-state.json schema mismatch
found while validating against a real cursor-agent install.

array parser can't be broken by either version. Bounds the sweep-
and manual-route-triggered tryIdeAuth() busy-timeout to 250ms
(down from the interactive auto-import path's 2000ms), since both
share the main event loop with all other in-flight requests and
should fail fast on a WAL-lock collision rather than block the
whole instance for up to ~4s. Has the manual refresh route bypass
the sweep's IDE-auth dedup cache so a click always sees a fresh
read, consistent with this plan's existing "manual actions never
see stale cached data" convention. Documents the previously-missing
agent-availability route in ROUTE_GUARD_TIERS.md's spawn-capable
