be65f94055 fix(db): renumber the synced-models migration to 177 and sync the doc count to 174
176 was taken on release/v3.8.51 by 176_xp_action_counts.sql (#12651), so the
migration runner aborted with a version collision on every DB open and took
down every DB-touching test. The column never shipped under 176, so no
retroactive guard is needed. The doc count moves 173 -> 174 across README.md,
AGENTS.md, llm.txt and its i18n mirrors (operator-approved numeric bump).

f7dbfc88c7 chore(tests): revive 51 vitest files parked behind a closed tracker (#13204) (#13233)
Merged after reconciling against the tip. `docs/architecture/QUALITY_GATES.md` conflicted: this PR adds the `check:vitest-exclusions` row while the tip had meanwhile rewritten the `check:model-lifecycle` description (#12535). Resolved to the tip's table plus the new row.

- `check:vitest-exclusions`: OK, 11 excluded files, each tracked and referenced
- ESLint over the changed files: exit 0
- `npm run test:vitest` (the config this PR edits): 472 of 473. The one red, `tests/unit/autoCombo/provider-family-combos.test.ts`, is not touched by this PR and passes 11/11 in isolation on both this branch and the pure tip — a load flake from the full run, not a regression.

fe73a3ad35 fix(responses): wrap forced-non-streaming web_search JSON as SSE (#13050)
Nasty shape: HTTP 200, upstream billed, and Codex times out waiting for `response.completed` that never comes because the body is JSON. Good catch that wiring the wrap in `handleResponsesCore` would not have fixed it — production goes through `handleChat`, and that helper is tests-only. Stamping the client's SSE intent before flipping `stream` off is the right place to remember it.

Reconciled against the tip after the batch landed: only the file-size baseline conflicted, resolved to the batch's rebaselined value. 5/5 afterwards.

c4eafaa26d fix(auth): probe a pinned inactive connection after quota top-up (#13017)
The one-shot framing is what makes this safe: a pin is an explicit operator act, so taking the inactive row only for that request, with siblings out of the pool and a 60s per-connection storm gate, keeps the blast radius at one request. Dashboard deactivate staying off is the right carve-out.

359b9b520b fix(db): self-heal under sustained memory pressure, VACUUM gate, WAL housekeeping (#13011)
A 36-minute outage with a full host-level write-up is the best kind of bug report. The WAL at 2.98 GB being the high-water mark a VACUUM leaves behind — rewriting a 3 GB database after deleting ~125 rows, with auto-checkpoint never shrinking the file and TRUNCATE only every 6 hours — is exactly the kind of thing that is invisible from inside the process, where the V8 heap read 300 MB while RSS was dominated by 1.9 GB of glibc main-heap.

Reconciled against the tip after the batch landed: the `ENVIRONMENT.md` table conflicted with #13035's `OMNIROUTE_SQLJS_WASM_PATH` row and both sides were kept.

ed2bf5dfe9 fix(nvidia): fail open when a synced model catalog goes stale (#12849)
A connection's synced model catalog (populated via Import Models, or opt-in
autoFetchModels/autoSync) was treated as authoritative forever once populated.
lookupModelMeta (src/sse/services/model.ts) rejects any model absent from an
authoritative synced catalog, and nothing ever refreshed it automatically
(modelSyncScheduler only re-syncs autoSync:true connections, off by default).
A NVIDIA connection synced once therefore had routing permanently pinned to
that moment's catalog: live upstream models added afterwards (even ones
present in the current static registry, e.g. moonshotai/kimi-k3) were
rejected with 'not available in the active live catalog' indefinitely.

Add a per-connection synced_models_at timestamp (provider_connections,
migration 176), stamped by replaceSyncedAvailableModelsForConnection on every
sync. getActiveSyncedCatalog now only treats a provider's synced catalog as
authoritative while at least one active connection was synced within
OMNIROUTE_SYNCED_CATALOG_STALE_AFTER_MS (default 30 days); once every
connection is stale — or was never synced, e.g. pre-migration rows — it fails
open the same way an unsynced provider already does, instead of gating on a
frozen point-in-time snapshot forever.

Root cause confirmed via a TDD repro proven RED against src/lib/db/models.ts,
src/lib/db/models/activeSyncedCatalog.ts and src/lib/db/providers.ts as they
stood on release/v3.8.51 (all 5 new assertions failed — the DB did not even
have the synced_models_at column yet), then GREEN after the fix
(tests/unit/nvidia-stale-synced-catalog-12849.test.ts).

The narrower reporter-blamed cause (a stale hand-maintained
open-sse/config/nvidiaHostedModels.snapshot.json allowlist) was already fixed
by #12538 and is not read by any runtime routing path.

#### `check-new-key-coverage` — new-key i18n gate
Sibling of `check-ui-value-drift`. That one catches an English value that was **rewritten**
while its translations were left behind; this one catches an English key that was **added**
while some locales never received it.
`check-ui-keys-coverage` cannot see this class: it enforces a percentage floor per locale, and
eleven absent keys out of ~13,000 leaves coverage at 99.9%. A percentage per language cannot
express "this feature shipped untranslated" — an entire feature can land in a new locale with no
text and never move the number.
The incident it encodes: Phase 3 of the Orchestration Canvas translated its eleven keys across
the 42 locales that existed at the time. Hours later the EU-language batch (#13044) took the repo
to 51 locales, and the nine newcomers (`el`, `et`, `ga`, `hr`, `lt`, `lv`, `mt`, `sl`, `sr`) never
received them. `deepMergeFallback` substitutes English for an absent key, so the failure mode was
untranslated UI rather than blank UI — real, and silent by construction.
Like its sibling it is **diff-aware**, comparing English at the merge base against the working
tree, so pre-existing gaps stay frozen and the gate needed no migration to turn on. Escape hatch:
`__MISSING__:<english>` defers a translation while keeping the runtime correct. `vi` bans
placeholders (`tests/unit/i18n-vi-completeness.test.ts`) and needs a real translation.

coverage to whoever reads the tree. Sixty-two files accumulated behind the comment
